Busy geriatric practice specializing in treating residents in assisted living facilities seeking social worker to work with our residents in facilities located in Harrisburg / Lancaster and York to coordinate all aspects of care, work closely with our intake coordinator and clinical staff to facilitate required paperwork, insurances, DME's, schedules and non-clinical needs. This candidate should appreciate working with the elderly population, be well-organized and feel comfortable making phone calls and using EMR. Must be willing to travel.
