POSITION SUMMARY- LSW

The hospital social worker will coordinate individualized discharge planning for patients in conjunction with family, the healthcare team and outside agencies. This position will ensure that well-coordinated patient centered care occurs, and the patent is able to transition care safely and timely. Also assists with placement for behavior health, assisted living, nursing home and swing bed. This position cares for patients of all ages from infant to elderly population.

POSITION QUALIFICATIONS

Education and Experience

Graduate of an accredited school and obtain a degree in social work.

Current licensure within the State of Minnesota required.

Experience in critical access hospital setting is preferred

Strong written and verbal communication skills

Demonstrated ability to assess and respond to patient and family needs in a timely manner

Basic computer skills, including working knowledge of Microsoft Office Suite, Epic electronic medical record keeping system, FindHelp program knowledge helpful.

Experience in establishing professional and collaborative relationships with patients, care providers and insurance providers.

ESSENTIAL DUTIES & RESPONSIBILITIES

Initiate assessment to determine patient’s needs, provide appropriate support to patient, family and significant others.

Attend care team rounds and communicate discharge planning information.

Review Advanced Directives and assist in writing them for patient when asked.

Knowledge of governmental regulations, guidelines and policies and work to ensure compliance for Medicare, swig bed program, skilled nursing facilities, assisted living, etc.

Assist patient and families with community resources and those seeking financial assistance.

Act as a patient advocate and ensure their rights are protected, and all patient information is kept confidential and HIPPA complaint.

Document discharge plan of care.

Provide emotional support to patients and families.

Other duties may be assigned as determined by business need.
