Optima Care Fountains is currently looking for a qualified professional with nursing home social service experience to fill an opening for a Social Worker with a privately owned long-term care facility located in Secaucus, NJ.
Responsibilities:

Assist with planning and implementing a comprehensive social services program which provides for counseling and other support services for residents and families in the facility;
Identifies medical-related social needs of residents, provides appropriate services to meet the individual, as well as collective needs of residents, maintains record relating to the residents' social work needs and care;
Works directly with residents and families' concerns related to the resident's physical and emotional condition;
Evaluates each resident's social needs then formulates plan for providing care;
Acts as a liaison between residents, families, outside agencies, and the facility to ensure that the resident's rights are maintained
Qualifications:
Bachelor's degree in Social Work or a bachelor's degree in a human services field including, but not limited to, Sociology, Special Education, Rehabilitatin Counseling and Psychology
Must have long term care experience
Must have excellent communication skills
Proven leadership and managerial skills
