Roswell Nursing and Rehab is seeking a caring and passionate Social Worker Assistant to join our social services team.
Roswell Nursing and Rehab is located on Green street, in the heart of the historic district in downtown Roswell. Parallel to Green street is the famous Canton street where sidewalk cafes, restaurants and boutiques line this charming street.
DUTIES:
- Identifying individual social and emotional needs
- Developing and Maintaining individualized care plans
- Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident.
REQUIREMENT:
- Previous experience working in long-term care preferred
- Bachelors degree in Sociology preferred
Salary: $18.50 - $23.00 hourly
