Brookhaven Center for Rehabilitation and Healthcare, a 122-bed skilled nursing and rehabilitation facility located in Essex County, New Jersey, is in need of Full Time Social Worker to assist the Director of Social Services. Candidate must have a current NJ Social Worker certification or license in good standing.

Duties include, but are not limited to:

• Develop and implement social service policies and procedures to meet residents’ needs in compliance with federal, state and local regulations.

• Develop, coordinate and participate in family and resident activities designed to promote social interaction, reality orientation and intellectual stimulation.

• Counsel residents and families in dealing with feelings about death or dying and other emotional, mental, environmental or physical limitations.

• Assess and complete the sections of the (MDS) Minimum Data Set assigned completely, timely and accurately.

• Document the social service component of the Comprehensive Care Plan for each resident in a timely manner.

• Document psycho-social needs, interaction and follow-up action(s) with resident and/or family according to facility policies and procedures.

• Assess, plan and document residents’ discharge needs in accordance with facility discharge planning policy and regulatory requirements.

• Act as a liaison with social, health and community agencies.

• Refer residents to social, health and community agencies and complete accurate documentation in residents’ records concerning the results of such referrals.

• Coordinate family and community resources as needed to solve financial needs and to promote emotional security.

• Explain residents’ rights to residents and residents’ representatives.

• Participate in admission of residents as assigned

• Plan, schedule and implement room visits for residents unable to leave their rooms.

• Organize and participate in the theft loss program.

• Be responsible for the safety of residents under his/her supervision.

• Demonstrates knowledge of and respect for the rights, dignity and individuality of each resident in all interactions.

• Appreciates the importance of maintaining confidentiality of resident and facility information.

• Demonstrate honesty and integrity at all times in the care and use of resident and facility property.

• Comply with all facility policies and procedures.

• Additional duties as assigned by the supervisor

QUALIFICATIONS:

• Must have long term care experience
• Must have prior experience as a Social Worker
• Must have LCSW or CSW
• Strong verbal & written communication skills
