SUMMARY:

The Social Worker is responsible for the counseling of assigned clients as well as their family members. The Social Worker acts as a resource person to other program staff and to the facility in the area of counseling, and family dynamics/interactions. The Staff Social Worker is under the direct supervision of the Administrator.

DUTIES PERFORMED:

• Conducts psychosocial history to determine pre-injury issues which need to be addressed.
• Establishes and carries out individualized treatment plan upon completion of psychosocial history if applicable.
• Provides counseling to clients, families, and/or significant others.
• Develops and implements family and children support groups as needed.
• Schedules arrange and moderate the program for the family education group.
• Participates in team rounds and family conferences as scheduled through the case manager.
• Maintains records and prepares reports according to facility procedures.
• Participates in and documents internal and external professional educational activities to enhance personal and professional growth.
• Performs public relations tasks as directed by supervisor.
• Coordinates client's schedule with physical therapy, occupational therapy, therapeutic recreation, nursing service, speech and language pathology, behavioral specialist, and other disciplines as needed.
• Is an integral part of the interdisciplinary team.
• Assists in patient discharge planning.
• Conducts post-discharge follow-up as needed.
• Any other tasks as assigned by the immediate supervisor.

SPECIFICATIONS

• Formal Education/Formal Training: Must hold a Degree in Social Work and applicable license/certification.
• Job-Related Experience: Minimum of two years experience in a health care setting, preferably an Inpatient Rehab Facility (IRF)
