What will you be Tasked with?
  • Performs comprehensive assessments in the patients’ homes and, identifies patient’s needs. Develops plan of care in collaboration with patient/CG’s, Health Care Provider, case manager and /or Supervisor as assigned and documents appropriately.
  • Assesses need for additional services and equipment on admission and on an ongoing basis and informs case manager and/or supervisor on a timely basis. Assesses home environment and make recommendations that will enhance patient care, promote safety and prevent adverse effects.
  • Works with patient, family and health care team members effectively to coordinate services, promote quality of care, and positive patient care outcomes. Revises and updates plan of care in collaboration with Health Care Provider, patient/CG as needs change and goals are met.
  • Takes verbal orders from Health Care Provider as necessary in a timely manner and documents appropriately.
  • Performs comprehensive assessments/reassessments of patients at admission/re-certification and as patient’s experience significant changes in condition. Completes OASIS accurately and timely, documents legibly and submits within 24 hours.
  • Advocates for patient’s needs by attending case conferences, in-services, and staff meetings. Coordinates plan for patient care outcomes through case conferences with other team members to provide appropriate, effective and efficient patient care and discharge planning and
    document appropriately.
  • Informs physician of changes in patient’s condition in accordance with physician’s orders and documents communications accordingly.
  • Performs LVN and CHHA supervision in accordance with state regulations, provides appropriate instructions to LVN and CHHA and documents accordingly. Provides clear, written and verbal instructions to LVNs based on their legal scope of practice, updates written instructions in a timely manner and documents appropriately.
  • Initiates appropriate preventive, rehabilitative, therapeutic, comfort, and pain interventions and provides instructions to patient/CG’s and staff in a timely manner.
  • Demonstrates professionalism by maintaining high standards of clinical care and staying current on clinical and medication issues, as well as, updates to patients’ clinical records including medication profiles, plan of care, OASIS, case conference, progress notes and re-certification documentations as required in a timely manner.
  • Cooperates and is reliable with making PRN, weekend and holiday visits to patients as assigned.
  • Informs supervisory personnel of any patient incidents promptly and documents appropriately.
  • Uses appropriate teaching materials and techniques to obtain positive outcomes and documents response to teaching accordingly.
  • Provides and maintains a clean safe working environment through observation of infection prevention and control and safety policies.
  • Acts as a preceptor when orienting new staff.
  • Provides and documents services in accordance with state, Medicare, Medi-Cal regulations, and agency policy as applicable.

Minimum Requirements:


Minimum Requirements

  • Graduation from an accredited nursing program, current state license as a Registered Nurse and CPR certified.
  • Has at least one year clinical practical nursing experience.
  • Must have vehicle for per diem or part-time. Will provide company vehicle if eligibility requirements are met.
  • Must have current driver’s license and automobile insurance.
  • Interpersonal and communication skill necessary.
  • Ability to make home visits and provide skilled nursing assessments and care in the home.

Preferred Requirements:
  • Home Health experience preferred.
