RN- Clinical Coordinator

Basic Job Function Summary:

Under the supervision of the Nurse Leader, coordinates home health care services, labs; and any other equipment, supplies, external services for the agency clients as prescribed by the physician, and/or requested by the client/family, and in compliance with the state’s Nursing Practice Act, any applicable licensure/certification requirements, and the organization’s policies and procedures. Additionally, responds to incoming and outgoing phone calls, physician communication; documents orders taken in a timely and accurate manner; is in frequent and timely communication with agency clinical staff related to clinical care, and coordination of care, assists in orientation, onboarding of new colleagues, assists in managing workflow, reviewing orders, overseeing scheduling processes, and resolving scheduling issues. This role also serves as backup to the Clinical Manager, when the Clinical Manager is out of the office. Some public/client relations responsibilities.

Essential Functions:

Quality of Care

  • Maintains a working knowledge of applicable Federal, State and local laws and regulations, the Organizational Integrity Program, Code of Ethics, as well as agency policies and procedures.
  • Ensures all clinical/client phone calls and service needs (including supplies, equipment) are addressed timely and per policy, processes, ensuring a high degree of patient/family satisfaction and care coordination
  • Obtains and accurately/timely documents necessary physician orders for home care services.
  • Reviews EMR workflow, manages, reviews Physician Orders, care coordination issues, follows up on QA documentation clarification, requests.
  • Provides timely follow-up on physician communication regarding lab results, medication/service changes, or other changes in the plan of care.
  • Responsible for assisting the Clinical Manager, scheduler and direct care staff in coordinating assigned patients/visits, and adjusting assignments, when required, based on emergent agency/patient needs, and productivity targets
  • Assists in the identification, development, and implementation of new/modified care coordination processes, programs/initiatives in order to efficiently meet client needs; while reducing risk and promoting safety and quality services according to regulatory and industry standards.
  • Assists in and proactively participates in projects/, huddles as assigned.
  • Assists Nurse Leader in planning/providing, overseeing orientation/onboarding of new clinical staff, including mentoring, making joint visits, and assuring new staff receive the orientation required for success.

Administration

  • Oversees, works collaboratively with Scheduler, to assure visits are scheduled per plan of care, with necessary changes made to accommodate patient service needs, and clinicians and patients are kept informed of changes to scheduled visit(s) .
  • Coordinates operations/schedules during non-business hours and holidays as assigned.
  • Assures that all required documentation is entered into the EMR system in a timely and accurate manner to facilitate concurrent interdisciplinary communication and quality patient care.
  • Functions as "back up" for CIM
  • Ability to supervise schedulers
  • Ability to act as "back up administer or DON of record

Communication

  • Ensures client care is coordinated with health team members
  • Notifies physician of changes in the client’s condition and/or need for additional orders, obtaining and documenting verbal orders per policy, as appropriate.

  • Keeps current in area of expertise and assures that standards and practices within the agency reflect best practice within area of expertise
  • Demonstrates furthering professional growth through sharing knowledge with others (e.g. presenting at agency and/or community in-services, acting as preceptor to others).

E. Customer Service

  • Identifies with and demonstrates a commitment to the mission and values of Trinity Health.
  • Ensures that services are provided in a manner that supports continuous quality improvement, customer-oriented focus and quality client care outcomes.
  • Able to effectively respond to customer complaints by maintaining calm professional attitude, (take the HEAT, Complaints are Gifts) and transferring to supervisor if unable to resolve quickly, effectively.

Minimum Qualifications:

  • Current RN licensure in the state in which practicing, strongly preferring a graduate of an approved BSN education program.
  • Three years or more of Home healthcare experience, or comparable experience in acute or ambulatory care. Healthcare supervisory experience preferred
  • Must have current Driver’s license and reliable transportation to and from work site.
  • Must be experienced in documenting in electronic medical records,
  • Must be proficient in the use of computer office software.
  • Ability to consistently demonstrate a commitment to the mission and Organizational Code of Ethics and adhere to the Compliance Program.
  • Must possess accurate and clear oral & written communication skills
