POSITION SUMMARY: The Restorative Nurse is responsible for coordinating restorative care to assist residents in attaining or maintaining the highest level of physical functioning possible for each resident without declines unless the decline is documented as unavoidable.

Restorative Nurse Requirements:

§ Licensed Practical Nurse (LPN) license in the state of Pennsylvania

§ 5+ years’ experience in restorative care

Restorative Nurse Responsibilities:

  • Plans, develops, organizes, implements, evaluates, and directs restorative services for residents.
  • Educates facility staff in restorative therapy and its execution in a 24-hour per day operation.
  • Maintains complete and accurate records of restorative service assessments and services.
  • Coordinates restorative therapy services with the therapy service providers to provide transitional services to facility residents.
  • Serves as the resident safety program coordinator, leading fall prevention, restraint reduction, and poly-pharmacy monitoring committees in accordance with State and Federal guidelines.
  • Possesses and demonstrates comprehensive knowledge of safe, restorative nursing care practices.
  • Evaluates and documents the effectiveness of the restorative care received by the resident.
  • Reports to the ADON and/or DON any adverse conditions experienced by the resident during the provision of restorative nursing care including, but not limited to, pain.
  • Monitors and assists with the performance of Activities of Daily Living (ADLs) to promote resident involvement to the highest extent possible without resident decline unless the decline is documented as unavoidable.
  • Evaluates and documents the resident’s progress and communicates results to the Nursing Department and seeks input and supervision to confirm findings.
  • Notes any unusual changes in resident's condition and reports to nursing staff.
  • Attends all mandatory in-services and participates in other in-services offered on a regular basis.
  • Reports all unusual circumstances that may involve the safety and wellbeing of the residents and co-workers, directly to the nurse on the unit, Nurse Supervisor, ADON, or DON.
  • Performs all other duties as assigned.
