LIMITED TIME $5000 SIGN-ON BONUS

Expected Skilled Registered Nurse (RN) Responsibilities:

• Receive and review patient medical diagnosis, prognosis, medications, procedures of case referrals to determine appropriate clinical course.
• Complete initial and ongoing comprehensive assessments of patients and families.
• Utilize health assessment data to determine nursing diagnosis.
• Perform skilled nursing visits and assist with private duty nursing coverage as needed.
• Facilitate coordination of care among interdisciplinary team.
• Assist clinicians in developing plans of care and guide clinicians to promote more effective performance and delivery of quality services.
• Conduct quarterly record reviews and communicate findings with Director of Nursing and Director of Operations.
• Supervise home care clinical personnel, including hiring and assisting in new employee orientation and coordination of clinical training.
• Participate in quality assessment performance improvement teams and activities.
• Ensure compliance with all state and federal legal and regulatory requirements.
• Participate in On-Call rotation every third week.
• Additional responsibilities as assigned.

Required from the Skilled Registered Nurse (RN):

• Registered Nurse with current licensure to practice in state of operation, BSN preferred.
• Wound care experience preferred.
• Infusion experience and port access required.
• PDN experience preferred.
• 2+ years management or supervisory experience.
• Excellent observation, nursing judgment and communication skills.
• Commitment to clinical and documentation excellence.
• Able to adapt to change with a positive attitude.
• Microsoft Word, Excel, and Outlook experience preferred.
• Clear Background Check.
• Satisfactory drug screen.
