Position Summary: Responsible for providing professional nursing care to patients following established standards and practices in the home setting. Functions as primary clinical liaison between patients and physicians.
Essential Functions:
- Performs initial and ongoing assessment of physical, psychosocial and environmental needs of patients in the home setting
- Confirms assessment findings with physicians and completes documentation
- Develops therapeutic nursing interventions to meet patient's individual goals and determines the need for additional skilled services
- Provides supervision for home health aide and licensed practical nurses as appropriate per the policies of HomeCare Maryland LLC and the Maryland regulatory agency
Qualifications/Requirements:
- Registered Nurse License for the state of Maryland
- Current American Heart Association BLS for healthcare providers
- Prior experience in home care preferred, but not required...positive attitude a must!
- 2-3 years as a Registered Nurse
- 2-3 years acute care setting experience
