Better Home Health Care, is a comprehensive home care agency in southern New Jersey. We are seeking full-time and part-time RN (Case Managers) to join our team.

As an RN, you will play an important role in our home health care services. In this position you will work with referring physicians to deliver direct care and provide the necessary support and education to patients and their families. Our nurses serve as liaisons for patients to evaluate and monitor their progress with the prescribed care plans and coordinating resources if needed. This role will allow you to work independently and utilize your home care experience and professional judgment to consistently meet best practice standards and established policies of the agency. This role will allow you to:

• Perform initial and ongoing compressive care assessments.
• Consult with patients and their families to identify care needs, develop care plans and identify short-term and long-term care goals.
• Teach self care to patients and families to ensure optimal results.
• Request home health aide services and write CHHA care plans.
• Available for emergencies, via phone call and/or patient visit
• Supervise the performance of home health aides and LPNs.
• Participate in home care on-call and rotate weekends and holiday call, as needed.

The successful candidates should be NJ-licensed registered nurses, preferably with a BSN, with at least one year of experience working in home health. Candidates will also need to have a valid driver’s license as this position requires traveling.

Job Types: Full-time, Part-time

Pay: $35.04 - $40.00 per hour
