The Quality Coordinator/RN is a registered nurse who functions in a leadership role in Population Health. They are a liaison between the patient and the multidisciplinary healthcare team to assist in developing and implementing a patient centered plan of care for high risk patients. The Care Coordinator assesses patients’ needs and coordinates resources within and outside the practice to promote optimal health benefits and quality outcomes in a manor most acceptable to the patient. They work in direct coordination with the local Managed Care Team and serves as an educational resource for providers. They also work with the Quality Department to analyze and communicate population health data to meet the Triple Aim of Population Health.

Qualifications*
Education: Graduation from an accredited nursing education program. Licensed by the State to practice as a Registered Nurse. Training in Motivational Interviewing preferred

Work experience*: Minimum of 5 years in case Management, Home Care or Hospice required

Knowledge*: Motivational Intervening, Shared Decision Making, Patient Centered Medical Home principles

Skills*: Certified Case Manager (CCM), Certifies. Critical thinking, problem solving, ability to work both independently and in teams, and ability to coach and mentor clinical and non-clinical staff

Abilities*: Operate within the concept of patient centered care

Equipment Operated*: Standard office equipment (laptop computer, fax, copier scanner)

Work environment:* Outpatient medical office setting

Mental/Physical Requirements:* Prolonged periods of sitting, multisite visits. Most work is telephonic
