Job Duties

Are you passionate about public health and creating healthier communities? The Western Tidewater Health Department is seeking a motivated a Public Health Nurse (PHN) to support Home and Community-based services provided by the Health Department. The PHN will work in the City of Suffolk and conduct home screening visits in partnership with Suffolk’s Department of Social Services. This position promotes public health and well-being of Suffolk residents, with a focus on the older adult population.

  • Displaying sound independent nursing judgements
  • Performing nursing assessments and administering case management.
  • Serving on a team involved with community engagement across the Western Tidewater Health District
  • Supporting collaboration with community based health care providers and community members
  • Provide support to public health emergency planning and response

The PHN will also support activities focused on assessing and monitoring the public’s health such as community health needs assessment and community health improvement planning. Must be knowledgeable of social and economic conditions related to public health and public health nursing. Actions performed at this level require licensure as a RN by state code. May supervise support staff.

Minimum Qualifications

  • Requires licensure as an RN in Virginia or compact state. If a new graduate, must obtain RN license within 90 days of the date of hire or subject to termination per the regulations governing the practice of nursing.
  • Requires CPR certification or ability to obtain certification.
  • Requires adherence to all safety policies including a review of immunizations, driver safety policies, and all other state policies.
  • Communicate verbally and in writing, analyze problems, situations and suggest solutions, leadership, follow directions and establish priorities as well as critical thinking skills
  • Ability to function independently, prioritize and organize work, solve problems, adapt to change, function as a team member and relate to the public and work with minimal supervision
  • Knowledge of public health concepts, principles of public health nursing, community resources and organizations, social and economic conditions, program administration and complex medical treatments, immunizations, basic nursing science, medical terminology and case management
  • Proficiency in computer functions including keyboarding, email, Microsoft Office and data entry
  • Required to maintain a valid driver's license and adhere to VDH's safety policy

Preferred Qualifications:

  • BSN, degree in health and human services or MPH.
  • Case management experience; home visitations, knowledge of public housing communities, experience working with the older adult population, public health experience and considerable nursing experience.
  • At least one year of med-surg or inpatient hospital experience.
  • Knowledge of LTSS policies and procedures.
  • Experience with long-term care screenings, DMAS or Virginia Medicaid processes
  • Willingness to travel between the districts four health departments
  • Graduation from an accredited school of nursing. Additional study in the area of health promotion/or public health is preferred
  • Experience in a public health setting or one related to disease prevention and health promotion

VDH requires all Health-Care Personnel (HCP) to provide documentation of immunization or evidence of immunity to the following: hepatitis B, measles, mumps, rubella, and varicella; a one-time dose of tetanus, diphtheria, and pertussis (Tdap) vaccine regardless of the interval since the last dose of tetanus-diphtheria (Td) vaccine was administered (Td or Tdap boosters shall be received every 10 years); and the current year's seasonal influenza vaccination.
