The Program Monitor (Quality and Care Coordinator) is responsible for monitoring key eliminates specific to Council on Accreditation (COA) standards, Patient Center Medical Home (PCMH) standards and guidelines, Uniform Data System (UDS) measures, and Healthcare Effectiveness Data and Information Set (HEDIS) measures pertinent to JeffCare

JeffCare, a program of Jefferson Parish Human Services Authority (JPHSA), is a Federally Qualified Health Center (FQHC) comprised of two Health Centers. JeffCare's goal is integration of primary care and behavioral health services to best serve the needs of the population; and, empowerment of individuals to achieve improved quality of life through the provision of effective services, delivered in an environment of integrity and respect. This position works closely with the Registered Nurse (RN) Clinical Quality Program Monitor assisting in the overall compliance responsibilities and provision of quality care within JeffCare, a program of JPHSA. This position is supervised by the RN Manager (Clinical Quality Program Monitor).

95% Quality of Care and Program Monitoring Specialist

  • Maintains a working knowledge of Council on Accreditation (COA) standards, Patient Center Medical Home (PCMH) standards and guidelines, Uniform Data System (UDS) measures, and Healthcare Effectiveness Data and Information Set (HEDIS) measures pertinent to JeffCare.
  • Assists the RN Clinical Quality Program Monitor in monitoring the quality of care provided in JeffCare as follows:
  • Monitors care of individuals served to ensure they are receiving the right service at the right time in the right place through EHR registries and chart audits;
  • Provides recommendations and implementation support when processes and procedures do not support improved health of individuals served;
  • Reports on staff compliance with JeffCare treatment protocols and how compliance or lack of compliance impacts client outcomes;
  • Monitors data required for performance indicators and other reporting requirements to ensure collected appropriately;
  • Oversees the entire process for JeffCare customer satisfaction surveys and PCMH required surveys: compiling the data, and delivering the data to the Quality and Care Coordinator for analysis;
  • Provides, compiles and assists with data collection for monthly Clinical Quality Measures (CQM);
  • Participates and assists with collection and review of data collected for JeffCare's annual UDS report;
  • Promotes ongoing supports of Council on Accreditation (COA) and Patient Center Medical Home (PCMH) certification by assisting with procedure development and data collection;
  • Monitors Healthcare Effectiveness Data and Information Set (HEDIS) measures, and provides feedback to the RN Clinical Quality Program Monitor regarding required documentation and/or services to improve JeffCare's measures;
  • Participates in multidisciplinary/integrated team huddles to update the team and problem-solve issues specific to COA, PCMH and HEDIS data collection and compliance;
  • Reviews reports as generated by JeffCare for quality improvement and appropriate care opportunities;

5% Completes other duties as assigned to meet the needs of JeffCare and/or JPHSA
  • Completes all required in-service/trainings in order to keep skills current as well as adhere to all JPHSA and JeffCare policies and procedures, accreditation standards, and civil service rules.
  • Participates in supervision per JPHSA Staff Development and Supervision Guidelines.


Physical Requirements:

  • The worker is not substantially exposed to adverse environmental conditions (such as in a typical office or administrative work).

Qualifications

MINIMUM QUALIFICATIONS:
A baccalaureate degree plus two years of professional level experience in administrative services, economics, public health, public relations, social services, health services, or surveying and/or assessing health or social service programs or facilities for compliance with state and federal regulations.
SUBSTITUTIONS:
Six years of full-time work experience in any field may be substituted for the required baccalaureate degree.
Candidates without a baccalaureate degree may combine work experience and college credit to substitute for the baccalaureate degree as follows:
A maximum of 120 semester hours may be combined with experience to substitute for the baccalaureate degree.
30 to 59 semester hours credit will substitute for one year of experience towards the baccalaureate degree.
60 to 89 semester hours credit will substitute for two years of experience towards the baccalaureate degree.
90 to 119 semester hours credit will substitute for three years of experience towards the baccalaureate degree.
120 or more semester hours credit will substitute for four years of experience towards the baccalaureate degree.
College credit earned without obtaining a baccalaureate degree may be substituted for a maximum of four years full-time work experience towards the baccalaureate degree. Candidates with 120 or more semester hours of credit, but without a degree, must also have at least two years of full-time work experience to substitute for the baccalaureate degree.
Graduate training with eighteen semester hours in one or any combination of the following fields will substitute for a maximum of one year of the required experience on the basis of thirty semester hours for one year of experience: public health; counseling; social work; psychology; rehabilitation services; statistics; experimental/applied statistics; economics; business, public, or health administration.
A Juris Doctorate will substitute for one year of the required experience.
A master's degree in public health; counseling; social work; psychology; rehabilitation services; economics; business, public, or health administration will substitute for one year of the required experience.
Graduate training with less than a Ph.D. will substitute for a maximum of one year of the required experience.
