The Program Coordinator is responsible for Support Plan implementation for each individual on their caseload. The Program Coordinator is responsible for the supervision of staff and the coordination of program activities within their program area.

Requirements:

Essential Functions

  • Provides direct supervision of Habilitation Instructors and Direct Support Professionals in assigned area.
  • Interviews and selects Habilitation Instructors and Program Assistants.
  • Coordinates the initial and ongoing training of staff.
  • Monitors the performance of staff, initiating and assuring prompt recognition and disciplinary action as appropriate.
  • Provides ongoing support and communication to staff.
  • Assures the completion of performance evaluations. (introductory and annual) for assigned staff.
  • Schedules staff to assure the proper coverage, approves time cards, and time off.
  • Conducts regular staff meetings for the purpose of communicating information, obtaining staff input, and providing training on operational and programmatic issues.
  • Coordinates and facilitate regular activities in Social Club time.
  • Assumes leadership in the classroom and provides training and support to Program Assistants.
  • Welcomes families, residential providers, volunteers, and other visitors in the facility.
  • Completes forms necessary for billing, including service summaries.
  • Coordinates Habilitation Activities for assigned area.
  • By regular observation and monitoring ensures a positive environment for individuals that allows learning and enjoyment of activities and that activities are meaningful and have purpose.
  • In conjunction with staff, develops habilitation activities for assigned area.
  • Ensures that direct support and training are provided to individuals.
  • Ensures that individuals are encouraged to be as independent as possible in all activities.
  • Ensures that individuals are properly supported in their personal care.
  • Ensures that curriculum lesson plans and activities are developed and modified as needed.
  • Ensures required client to staff ratios are maintained in each classroom and serves as qualified staff within that ratio the required minimum or as needed to assure services are billable.
  • Ensures implementation of all relevant components of the Individual Support Plan (ISP) for each assigned client and ensures that performance data is collected as prescribed.
  • Ensures the implementation of Behavior Support Plans (BSP) as written for individuals, providing on going supports and completing data collection and/or observation reports as required. Models and teaches effective proactive / reactive strategies to staff.
  • Ensures that service documentation and data collection are completed as required.
  • Reviews incident reports, conducts incident investigation and makes recommendations for action as appropriate. Completes BDDS reportables as required.
  • Welcomes families, residential providers, volunteers, and other visitors in the facility.
  • Ensures that supplies and equipment are maintained, repaired and purchased as needed.
  • Reports maintenance, housekeeping and IT needs to appropriate staff.
  • Facilitates volunteer opportunities within the program.
  • Provides case coordination for assigned caseload of individuals participating only in LOGAN Studios.
  • Develops and maintains primary relationship with individuals; their families, guardians and advocates and other providers.
  • Coordinates and facilitates all functions of the IST, including the PCP/ISP, for individuals on State Line Funding.
  • Serves as agency representative for all services at all meetings of the IST.
  • Serves as agency liaison with Case Managers and group home providers and ensures receipt of all necessary documents.
  • Works with the IST to ensure adequate resources are in place for services rendered, reviews budgets for accuracy and works with others as needed to complete required corrections/modifications.
  • Ensures specific goals for the individuals are developed, implemented, and monitored and teaching strategies are modified on a monthly basis or more often as needed. Completes required documentation and sends to members of the IST.
  • Coordinates the development and implementation of risk plans and positive behavior supports for all individuals.
  • Facilitates review of Behavior Support Plans and rights restrictions by the Human Rights Committee as appropriate and provides updates as required. Communicates pertinent information to the Human Right Committee as needed.
  • Maintains electronic master file.
  • Works cooperatively with individuals served, families, outside agencies, and LOGAN.

General Responsibilities

  • Assists in promoting the health and safety of individuals served by LOGAN, visitors, and other staff.
  • Serves as role model for individuals served and other staff.
  • Serves as an advocate and protects the human rights of all individuals served.
  • Participates in training as required by supervisor or for all LOGAN staff.
  • Facilitates professional growth by keeping current on new philosophy, research, technology, program strategies, etc.
  • Responsible to complete other duties as assigned by supervisor.

QUALIFICATIONS

Education

  • High school diploma or equivalent required.
  • Bachelor’s degree in related field preferred.

Experience

  • Two years experience required. Experience may include a combination of life experience and working with individuals with developmental disabilities.
  • Supervisory experience preferred.

Job Knowledge

  • A positive attitude to support individuals with disabilities.
  • A willingness to learn and implement positive approaches to instruction and behavior management.
  • Competent computer skills.
  • Ability to effectively communicate orally and in writing.
  • Ability to lift a minimum of 50 pounds.
