Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Program Coordinator at our Worcester site.

The Randolph House program residential program serves 8 adult clients with persistent mental illness and is staffed 24/7. This program prides itself on fostering a family-like atmosphere by providing a nurturing, safe, supportive, and respectful milieu. A dedicated team of staff work collaboratively with providers, administering education and support to each client to ensure that they meet their individual goal.

The program provides support such as medication management, symptom management, relapse prevention, transportation to appointments, healthy meals, and a weekly staff-lead skills or recreational group. Staff work with individuals on their specific needs and attend team meetings and collaborate with providers. This position provides an opportunity to join a team of dedicated staff who provide ongoing support and services to individuals with persistent mental illness.

The Program Coordinator oversees the operation of the residential component consistent with the administrative, programmatic, and clinical polices of Community Healthlink and its Recovery Environments and Supports Division.
Responsibilities:
  • Performs duties using recovery orientated principles.
  • Ability to hire, train, supervises and evaluates program staff and ensures required competencies are maintained.
  • Involvement with the clinical team operations including behavioral and rehabilitative treatment planning for individuals with mental health and substance abuse issues.
  • Demonstrate person-centered and strength-based communication and relationship with Persons.
  • Demonstrate effective communication with staff, peers and other professionals.
  • Operate program in an efficient and effective manner ensuring compliance with BSAS, HUD, DPH and DMH regulatory requirements.
  • Provide oversight of the residential component.
  • Act as administrative site manager during his/her absence.
  • Ensure operational budget for the program is reconciled.
  • Orient and train new staff, as designated, to daily operations of the program and Division procedures.
  • Ensure new staff are oriented and trained as required by CHL.
  • Provide ongoing supervision to program staff.
  • Facilitate the transition for referrals, admissions and discharges.
  • Strive to maintain maximum capacity.
  • Collaborate with the assigned LPHA to Plan, implement and continuously evaluate rehabilitative and support services through assessments and treatment planning to meet individualized needs, and development of a peer support group.
  • Take personnel action regarding hiring and disciplinary procedures.
  • Participate in administrative on-call through the emergency on-call system.
  • Serve as liaison among other departments and with other agencies to facilitate a continuum of care.
  • Identify problem areas and coordinate work for upkeep of site. Insure day-to day upkeep of residence and workplace.
  • Work with Persons and staff daily to maintain the residence including chores, bedroom upkeep, yard and other housekeeping duties.
  • Ensure accuracy and timeliness of Persons calendar, admissions, discharges and transfers.
  • Ensure healthful menu planning, cost effective food shopping and meal preparation through coordination and assistance.
  • Perform other related job duties.
  • Transporting Persons in personal car and/or program vehicle.
  • Monitor Persons Medications, refills, documentation of medications and changes, disposal of medications, assist with attending appointments, and safe storage.
  • MAP, CPR, First Aid certifications must be completed within the first five months after hire date.

Qualifications:
  • Bachelor’s degree in social work or related human services field plus 2 years of experience.
  • Will consider several years' experience in lieu of college degree.
  • Must have supervisory experience
  • Current valid US-issued driver's license and a registered inspected and insured automobile for work related purposes including transporting clients in a personal vehicle.
