The Harmony Healthcare Long Island, formerly known as (Long Island Federally Qualified Health Center or LIFQHC) is a non-profit healthcare organization with 7 health centers, providing primary care and preventative medicine in the following locations in Nassau County: Roosevelt, Elmont, Hempstead, Freeport, Oceanside, and New Cassel/Westbury. In addition, the Harmony Healthcare Long Island has 3 school-based health centers, WIC offices (Special Supplemental Nutrition Program for Women, Infants, and Children) in 3 locations, and a Health Home Care Coordination program. As federally qualified health centers, we serve the individuals in our communities, providing enhanced services, expanded hours and reduced prescription pricing, while raising the level of care. JOB TITLE: Podiatrist


REPORTS TO: Medical Director


The following statements reflect the general duties, responsibilities and competencies considered necessary to perform the essential functions of the job and should not be considered as a detailed description of all the work requirements of this position.


RESPONSIBILITIES:

  • To take medical histories when appropriate and perform as part of an interdisciplinary team, make an initial clinical assessment of addition and a recommendation to the Director regarding the patients’ appropriateness for care
  • To monitor progress in and appropriateness of treatment, specifically regarding dosage and medication (if any)
  • To perform physical examinations and provide medical assistance as indicated by policy and patient needs, and to order and review lab work as appropriate
  • To be available for consultations regarding patient care, and make referrals when indicated
  • To participate as necessary in utilization review meetings
  • To plan and carry out patient education sessions on medical issues referable to
  • To be available to respond to requests for information and performing outreach activities
  • To be knowledgeable and fully compliant with general standards of departmental job descriptions and all policies and procedures that govern the operations of the organization
  • Performs other duties as directed and necessary
  • Review patients’ charts
  • Participate in committee activities
  • The identification, assessment, reporting and referral of cases of suspected child abuse or maltreatment and identification and treatment of victims of domestic abuse
  • The identification of patient’s medically related personal and social problems which may interfere with the patient’s treatment, recovery, or rehabilitation


MINIMUM QUALIFICATIONS:

  • New York State License
  • If applicable, Board Eligible or Board Certified in Specialty Field
