Amity Rehab Services LLC is currently looking for Physical Therapists, Occupational Therapists, Physical Therapist Assistants and Occupational Therapy Assistants to provide care for patients in the home health setting serving Greater Houston and all surrounding areas. We require coverage in all following areas:

League City, Pasadena, Pearland, Sugar Land, Cypress, The Woodlands, Baytown, Conroe, Deer Park, Friendswood, Missouri City, Rosenberg, Texas City, Lake Jackson, LaPorte, Atascocita, Kingwood, Channelview, Mission Bend, Spring, Alvin, Angleton, Bellaire, Clute, Dickinson, Freeport, Galena Park, Humble, Jacinto City, Katy, La Marque, Richmond, Santa Fe, Seabrook, South Houston, Stafford, Tomball, Webster, West University Place, Aldine, Cinco Ranch, Cloverleaf, Four Corners, Fresno, Greatwood, New Territory, Pecan Grove, Sienna Plantation

Essential Duties and Responsibilities: PT/OT

  • Perform initial and ongoing clinical assessments to identify patient needs, determine level of functioning, including OASIS assessments at appropriate time points and appropriateness of patient for home health.
  • Develop and revise a Plan of Care in consultation with the physician and other care-team members.
  • Provide therapeutic treatments and evaluation of equipment needs as applicable.
  • Evaluate outcomes of care with appropriate discharge planning.
  • Apply concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
  • Accept clinical assignments that are consistent with education and competence to care for patients.
  • Supervise PTA and HHA as appropriate.
  • Complete appropriate documentation in a timely manner.
  • Meet mandatory continuing education requirements of the agency/licensing board and participate in in-service programs.
  • Use effective interpersonal relations and communication skills to advise and consult with the family and other agency personnel as appropriate.
  • Promote Agency philosophy and administrative policies.
  • Demonstrate commitment, professional growth and competency.

  • Performs services planned, delegated and supervised by the PT
  • Uses effective interpersonal relations and communication skills to participate in teaching the patient and family
  • Completes appropriate documentation in a timely manner
  • Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease
  • Accepts clinical assignments that are consistent with education and competence to care for patients
  • Meets mandatory continuing education requirements; participates in in-service programs
  • Demonstrates commitment, professional growth, and competency

Qualification Required:

  • All Physical Therapists, Occupational Therapists, Physical Therapist Assistants and Occupational Therapy Assistants must hold a current license to practice in the State of Texas and meet all eligibility requirements set forth by prospective Boards in Texas.
  • Must provide valid and current automobile liability insurance
  • Must provide a current CPR card
  • Valid Driver’s License
  • Ability to perform work that requires frequent standing, bending, reaching, squatting, kneeling, moving, lifting of patients and/or equipment up to 50 pounds
