Aleca Home Health is a growing and busy Home Health agency providing high level residential healthcare to patients throughout Kitsap County. Aleca Home Health is a division of the Encore Communities/Alumus~Sante’ Holdings, LLC family with our office located in Silverdale, Washington.

We take pride in being a Medicare & CHAP certified Home Health agency committed to providing “Care with Compassion” and fulfilling the needs of the community to the best of our ability by providing a variety of services to patients in the comfort of their residence. Our services include nursing, physical, occupational and speech therapy, social workers and home health aides. Specialized programming is available for those with COPD, pain, and congestive heart failure.

Job Summary

The Physical Therapist is responsible for the evaluation, planning, directing and administering of physical therapy modalities of treatment as prescribed by a licensed physician; administers treatments and physical agents as prescribed by a referring physician in an effort to restore function and prevent disability following injury, disease or physical disability; and, assists patients to reach their maximum performance and level of functioning, while learning to live within the limits of their capabilities.

Candidate Profile

• Graduate with a minimum of B.S. from a physical therapist curriculum required.
• Current PT license through the OR State Board of Physical Therapy required.
• A minimum of 2 years PT experience required.
• A minimum of 1 year Home Health PT experience preferred.
• Wellsky/Kinnser experience preferred.
• Valid driver's license with proof of insurance in accordance with state requirements. 100% travel required.
• COVID-19 vaccination required, or reasonable accommodation based on disability, medical condition, or religious belief that prevents vaccination.
