PACE, a member of Empath Health is currently seeking a Physical Therapist (PRN) to join our team.

The Physical Therapist:

• Under the supervision of the PACE Senior Physical Therapist, provides physical therapy services to participants of Suncoast PACE
• Provides clinical physical therapy evaluation and treatment for program participants, on acute, restorative or maintenance levels as needed by Day Center, ALF, Nursing Home or in-home setting.
• Integrates the physical therapy treatment plan into the Interdisciplinary Team comprehensive plan of care.
• Assists in the ordering, inventory, distribution and maintenance of durable medical equipment for participants.

Position Requirements

• Be a graduate of a physical therapy curriculum approved by one of the following: The American Physical Therapy Association; The committee on Allied Health Education and Accreditation of the American Medical Association; The council on Medical Education of the American Medical Association and the American Physical therapy Association required. Other organizations approved by the Secretary.
• Licensure:

o Current State of Florida Physical Therapy license

o Valid Florida State Driver’s License

• Certifications:

o Valid CPR certification from the American Red Cross or American Heart Association.

• For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.
