Responsibilities:
The Physical Therapist will work Full Time, Days and will evaluate, establish treatment plans, and provide physical therapy to patients in the hospital, nursing home and out-patient setting. The PT Oversees the services provided by the Physical Therapy Assistants and Aides.
  • The Physical Therapist performs a comprehensive physical, psychosocial, and safety assessment of assigned patients as evidenced by clinical documentation.
  • Selects and performs therapeutic techniques that are appropriate to patient's problems and medical status, and that reflects integration of the therapist's clinical knowledge and theory.
  • The Physical Therapist will document client status and progression towards achievement of outcomes on each visit in measurable parameters.
  • The Physical Therapist will develop a Plan of Care to promote optimum function and safety as documented in the clinical record.
  • The Physical Therapist establishes interventions and specific measurable outcomes that are consistent with identified problems.
  • The Physical Therapist modifies and updates the plan of care as patient status warrants.
  • Coordinates patient care with other clinical team members ensuring optimal patient care and communication as noted in the clinical record.
  • Completes all required paperwork to ensure that documentation reflects care provided and the outcomes achieved.
  • Maintains and enhances personal knowledge as well as promoting professional standards in physical therapy.
  • Demonstrates respect and regard for the dignity of all patients, families, visitors and fellow employees to ensure a professional, responsible and courteous environment.
  • Acts as a clinical education instructor for students who come from a variety of settings including independent or school-arranged student observations, New Visions, PTA, and PT programs.
Qualifications:
Bassett Healthcare Network follows the New York State and New York State Department of Health COVID-19 vaccine mandates for healthcare workers. This mandate requires COVID-19 vaccinations for personnel working for or on behalf of our hospitals and nursing homes. The New York State mandate allows for medical exemptions and religious accommodations, when appropriate. All candidates who accept an offer from Bassett Healthcare Network will be required to show proof of vaccination as required by New York State.

Education:
  • Bachelor of Science degree in physical therapy from an accredited college or university and eligibility for the New York State licensure examination.
Experience :
  • Experience as a Physical Therapist prior to employment is desired. New graduates will be considered.
Licensure/Certificates:
  • A valid temporary permit or license granted by the New York State Department of Education is required, with the following exception as allowed by the New York State Office of the Professions:
    • New graduates of New York State or APTA accredited physical therapist education programs registered by the New York State Education Department may be employed for 90 days immediately following graduation without holding a limited permit. To be eligible to practice within these 90 days, you must have:
    • Graduated from your physical therapist program and requested your school to complete and submit the Form 2 to the Office of the Professions;
    • Submitted the Application for Licensure (Form 1) and paid the required licensure application fee
    • Submitted the Application for Limited Permit (Form 5), and paid the required limited permit fee, and
  • Applied to take the licensing examination and paid the fee for the examination
Skills:
  • Must be capable of understanding and carrying out prescribed therapy services as requested by physicians and/or midlevel providers or as otherwise governed by the NYS Guidelines for Physical Therapy practice.
Physical Requirements:
  • This position requires frequent standing, sitting, and walking, as well as occasional lifting, kneeling, and bending.
  • The ability to transfer patients to various surfaces is also required.
