We are a growing company seeking a quality Physical Therapist dedicated to excellent care. The hours are PRN, and the schedule is very flexible. Interest in home health settings is required.
Previous experience is preferred.

Our company covers home health therapy in the majority of northwest Indiana. We provide therapy for patients in St. Joe and Elkhart counties in addition to the lake area, and our therapists have the privilege of designing their own schedules.

A PT in this position will be accountable to our rehab director and coordinate with office staff on their weekly schedules.

Responsibilities include:

• Establishing, monitoring, and delivering physical therapy to patients/clients, as directed by the physician’s plan of treatment and the interdisciplinary patient/client plan of care.
• Performing physical therapy assessments and procedures which require substantial specialized knowledge, judgment and skill based upon principles of psychological, biological, physical and social sciences.
• Making decisions based on individual’s educational preparation and experience in Physical therapy.
• Reporting on the condition of patients to the appropriate supervisor, and other staff members.

We are looking for a Physical Therapist to join our team and provide home health services to our clients. The ideal candidate will have experience in home health physical therapy, be highly organized and have excellent communication skills. The Physical Therapist will be responsible for assessing patient needs, developing treatment plans, and providing physical therapy services in the patient's home. The successful candidate will also be responsible for providing education and support to the patient and their family.

Responsibilities:

• Develop and implement physical therapy treatment plans for home health patients.
• Assess home health patient's condition and needs.
• Educate patients and family members on proper physical therapy techniques.
• Provide patient with exercises to improve strength, flexibility, and range of motion.
• Monitor patient progress and adjust treatment plans as needed.
• Document patient progress and outcomes.
