Summary
This position is responsible for attending to the daily living and safety needs of an assigned group of participants and aiding the licensed nursing associates in the care of participants by performing various participant care activities and related non-professional services

Essential Duties

  • Assists participants with activities of daily living, such as feeding, ambulation, and personal hygiene generally in the setting of the PACE center.
  • Collects data and records activity and participant care information in the EHR. Ensures that information is accurate, timely, and recorded according to established guidelines.
  • Provides homemaking services including preparing and serving meals and snacks according to specified nutritional needs and dietary restrictions as outlined on the plan of care.
  • Observes participants for emotional, behavioral, and physical changes.
  • Assists the AllCare PACE team in a variety of activities to serve members.

Education & Experience
High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.

Certificates, Licenses and/or Registrations

 Current BLS Certification.
 Current State of Oregon Certified Nursing Assistant license.

Language Skills
Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.

Mathematical Skills
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Computer Skills
Has advanced basic computer job skills including logging on to systems, ability to communicate by email, ability to compose documents, enter database information, create presentations, download forms, and preserve/backup important data.
Reasoning Ability
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to work with problems involving several concrete variables in standardized situations.
Interpersonal Skills

Physical Demands & Work Environment
The employee must occasionally lift and/or move 10-50 pounds. This position has exposure to communicable disease and body fluids. While performing the duties of this job, the employee is regularly required to sit with frequent walks of short distances; drives among medical system facilities; uses hands to finger, handle, or feel and talk or hear. The noise level in the work environment is usually moderate.
