EDDY VISITING NURSE & REHAB ASSOCIATION

Eddy Visiting Nurse & Rehab Association has a rewarding opportunity for a per diem Social Worker responsible to manage a caseload of patients requiring social work support through in-home visits. Some patients/families are supported as well through virtual/phone based social work support either in place of in home visits or in combination with in home visits.

Manages a caseload of patients in their homes:

• work with an interdisciplinary care team of nurses, therapists and aides
• conduct a comprehensive assessment of patient/family needs
• link individuals to the community resources they need to remain in their home or move to a higher level of care if necessary
• Once trained, enjoy the independence of starting and ending your day from home with a flexible work schedule
• Be on the front-line of the transitions in health care from inpatient settings to the community – home health care is the future!

Work schedule is flexible.

Requirements:

• NYS Social Work Licensure, minimum of 1 year of relevant experience in home care, the medical field, work with seniors, etc . required.
  

Eddy VNA provides a comprehensive orientation to home health care. Eddy VNRA provides a supportive work environment with access to social work supervision, in-depth computer training, assistance obtaining insurance authorizations, support staff who are only a phone call away, and more!
