MEDICAL SOCIAL WORKER-Hospice
Lorian Health is an established home health agency seeking energetic members to join our team of professionals in San Diego, CA. Lorian is currently looking for a top notch Medical Social Worker (MSW) to provide optimum care to our Hospice patients in San Diego County.
Major responsibilities include assessing and evaluating patients for their need of community resources and in home support services, assisting in the implementation of the plan of care, assisting patients with advance directives, and preparing and submitting accurate clinical notes. Our visit notes and medical records are 100% electronic allowing you to complete all of your charting inside the patient’s home.
What We Offer:
We are a pay per visit compensation model. Per visit rates are $80-$100, depending on type of visit.

To be considered, all applicants must be licensed in the state of California by the Board of Examiners. One year of social work experience in a health care setting required.

What You Will Be Doing: *
Job Summary

The Hospice Medical Social Worker’s responsibilities will include assisting patients living with terminal conditions during their end of life period. Arranges for care to enhance the patient’s qualify of life and counsels the patient and his family as they adjust to the diagnosis. Being an advocate for patients and their families in and out of the hospice setting. Evaluating the patient’s psycho-social and emotional condition, needs, resources and family situation. Also considers the goals of the patient and their family. This position will assist in the implementation of the plan of care; assisting patients with advance directives, and preparing and submitting accurate clinical notes. Lorian Hospice visit notes and medical records are 100% electronic allowing for this position to complete all charting inside the patient’s home.

Responsibilities*

Care coordination. Hospice social workers coordinate the care of patients with a Hospice Interdisciplinary team of medical and patient service professionals. Assisting the physician and other IDT members in understanding the significant social and emotional factors related to the patient health problems. Participating in the development of the plan of treatment or Plan of Care (POC).

  • Psycho-social assessments. Hospice care social workers conduct assessments of clients and their families to determine their psychological, social, emotional and spiritual needs.

Counseling and psychotherapy. Hospice care social workers also provide emotional support, counseling and psychotherapy to clients and their families who are experiencing psychological and emotional difficulties with their loved one’s prognosis

Crisis intervention. Social workers in hospice care settings may also intervene and provide emergency psychological support when clients or their loved ones are experiencing mental, emotional, social crisis situation.

Patient education and resource navigation services. Hospice care social workers help clients and their families understand their treatment plan and the processes involved in hospice and palliative care.

Observing, recording, and reporting information on the patient’s community resource needs to the DPCS, case manager, and in the patient’s health record.

Helping the patient and family make end-of-life plans such as funeral arrangements and/or establishing an advanced directive.

Participating in discharge planning.

Participating in in-service education programs.

Qualifications

Graduate of an accredited school approved by the Council on Social Work Education.

  • Master’s degree from a school accredited or approved by the council on Social Work Education.
  • One (1) year experience as a MSW.
  • Possesses and maintains good physical stamina and mental health. Presents a pre-employment physician’s health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statute.
  • Has the ability to read and follow written instructions and document care given.
  • Is self-directing and has the ability to work with little direct supervision.
  • Possesses and maintains current CPR certification.
  • Is flexible and cooperative in fulfilling the role obligations.
  • Demonstrates tact, patience, and good personal hygiene.
  • Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements.
  • Must be able to verbally communicate in the language of the client and document all client related activity practices.
