The Center for Hope Hospice & Palliative Care is a unique Union County-based nonprofit organization, seeking to hire a Coordinator for the Patient Services department.

Key Responsibilities:

The Patient Services department covers the following divisions: Intake, Scheduling, Volunteer, Social Services. Spiritual. The Patient Services Coordinator (PSC) is responsible for various duties to help the department meet its daily goals and objectives efficiently and effectively. The PSC assists the department coordinators with organizing and prioritizing tasks while providing administrative functions within the department. While a majority of the PSC's time will be spent assisting with Intake, the position also ‘floats’ as needed to help support other key task areas within the department.

JOB ROLE & RESPONSIBILITIES:

  • Covering the critical role and responsibilities of the division's coordinator during call-outs and PTO.
  • Providing administrative support, as needed, to all the department divisions
  • Answers call and take messages for the department and especially the Intake division.
  • Provides follow-up calls regarding referral status and educates callers on hospice and palliative services
  • Daily use of an Electronic Record (EHR), Microsoft Office, and web-based software.
  • Monitors the division’s outlook emails, portals, and electronic fax accounts.
  • Maintains our daily intake list/report
  • Obtains and processes intake referrals, including patient demographics, insurance information/verification, and other pertinent chart information as needed for chart completion.
  • Responsible for notifying employees or agency staff regarding available shift changes/openings
  • Assist with volunteer process and screening to increase volunteer participation
  • Responsible for updating and organizing the department's database and record keeping, including policies and procedures

Qualifications

  • Excellent customer service skills
  • Excellent written communication and verbal skills ( English speaking )
  • Ability to multitask, accurate data entry and typing speed 50+ WPM
  • Proficient in Microsoft Office, operating computer programs, copying, faxing, and scanning.
  • Highly organized, able to follow processes and procedures
  • Effective time management and organizational skills
  • Experience in Healthcare/understands medical terminology
  • Hospice, Palliative care or medical background- LPN/CNA ( preferred, not required )
  • Bilingual (preferred, not required)

Schedule:

  • Day shift/ Monday to Friday
  • Some holidays will be required
  • Onsite to start (and a hybrid schedule will be available in the future and based on performance)
