Summary

Provides coordination and administrative support for all aspects of patient care, including building relationships with patients, problem solving in partnership with leadership and ensuring all medical information is available to patients throughout their experience.

Responsibilities

Coordinate all aspects of patient care including but not limited to:

  • Scheduling clinic appointments, patient referrals and other internal and external providers as needed
  • Obtaining patient prescription orders and refills
  • Assisting with durable medical equipment and prosthetics
  • Responding to patient inquires in a timely manner (within 24 hours) and updating shared documents accordingly
  • Serves as the main intake personnel
  • Working closely with the clinical and finance team to ensure patient receives all information necessary to proceed with treatment


Assist patients in appointments, surgeries and consultations, including services such as interpretation as needed; including coordinating transportation and booking housing accommodations.



Accurately collects and enters all necessary patient information including but not limited to:

  • Letters of medical necessity from medical providers
  • Insurance verification
  • Cost estimates from finance
  • Media consents
  • Collecting all patient surgery clearances and coordinating surgical times


Support strategic institute initiatives to improve patient care including but not limited to: research, patient education, relationship building, etc.


Education

Required: Minimum LPN, Associates Degree in Nursing or related field or commensurate clinical/patient experience
Preferred: Bachelor Degree

Experience

Required: Minimum 5 years experience in acute care setting as a Licensed Practical Nurse, Case Manager or Social Worker
Preferred: 7 years preferred. Conversant knowledge and experience with case management, care coordination or discharge planning.

Certifications

Required: Must be currently licensed, certified or registered to practice profession/vocation as required by law, regulation in state of practice or policy or Case Management Certification
