Aledade Care Solutions is a new business unit at Aledade charged with designing, testing, and deploying new clinical service lines and business offerings that will augment and extend primary care practices’ clinical capacity, offering selected patients a portfolio of virtual and (eventually) home-based services to improve health at lower costs.

We are searching for a Patient Outreach Analyst to join the team. This analyst will be responsible for organizing and manipulating multiple data sources as well as leveraging this data to deliver actionable insights for key stakeholders. Specifically, the Patient Outreach Analyst will be responsible for the creation and dissemination of reporting that encompasses agent level outcomes, patient outreach outcomes, and status updates as requested. Eligible candidates must be comfortable collaborating across teams, organizing data, identifying trends and managing multiple data sources. Ideal candidates will possess strong verbal and written communication skills, be detail oriented and fluent in Microsoft Office and Google Suite applications.

Responsibilities:

    • Pull, combine and display Patient Outreach data from multiple data sources including, but not limited to the Aledade App, call center telephony platform, and other data sources as needed
    • Organize and manipulate data to be presented to end users in an actionable manner including the creation of dashboards
    • Use Patient Outreach data to identify and deliver actionable insights to key stakeholders including trends in patient outreach engagement rate, scheduling conversion, and new opportunities for improvement
    • Identify, monitor and communicate changes and trends in outreach efforts which affect key metrics
    • Establish relationships, gather input and facilitate communication amongst key stakeholders
    • Review and research data discrepancies across sources to sustain data integrity

Required Education, Skills, and Experience:

    • Bachelor’s degree in related field or equivalent work experience
    • Minimum 3 years of experience with data management and reporting
    • Highly organized individual
    • Proficient in Microsoft Excel and Google Suite Tools such as Google Sheets
    • Ability to work with various software applications daily
    • Ability to effectively communicate verbally and in writing
    • Effective collaboration skills across teams
    • Proactive and self-motivated individual
    • Comfortable working in a fast paced environment
    • Ability and comfort with tackling and prioritizing multiple tasks

Preferred Experience:

    • Minimum of 1 year of experience with data management and reporting in a call center environment
