Posting Summary
Rutgers, The State University of New Jersey is seeking a Patient Navigator for Medicine-Infectious Disease Practice within the New Jersey Medical School.

The primary purpose of the Patient Navigator position is to provide clinic and community-based HIV, STI testing, and case management services to persons with and at risk for HIV infection. Performs a biopsychosocial assessment and develops an informed care plan to address any identified social service needs and treatment adherence barriers. As part of the team, the Patient Navigator will provide services in clinical settings and on the mobile health unit including working select weekend and evening hours. Maintains the confidential integrity of all client level information. Proactively reports any issues requiring action, intervention, or resolution to the Program Manager in a timely manner.

Among the key duties of this position are the following:
  • Orients clients and provides information about the available services of the ID Practice.
  • Performs a biopsychosocial assessment and develops a care plan to address the client’s needs initially and at a minimum every six months.
  • Coordinates the activities of physicians, clinical staff, patients and families and other agencies to ensure optimal patient outcomes.
  • Assists patients diagnosed with HIV to navigate the healthcare system. Assists patients in overcoming barriers within the health care system to ensure adherence to medication regimen and appointments. Escorts patients to dental and other services.
  • Assists physicians with all secretarial/coordination aspects of patient care for patients and follows physician protocols according to diagnosis and referral.

Qualifications

Minimum Education and Experience
  • Bachelor’s Degree in Social Work or Human Services plus two (2) years (five (5) years preferred) of full-time professional experience with HIV/AIDS.
  • Equivalent education, experience and/or training may be substituted for the degree requirement.

Certifications/Licenses
  • NJ certification as a HIV Counselor and Phlebotomy certification required or must be prepared to obtain within six (6) months of employment.
  • Valid Driver’s license.

Required Knowledge, Skills, and Abilities
  • Completed or be eligible to successfully complete the Rutgers Defensive Driving Training Course.
  • Must be able/willing to learn to drive mobile unit.
  • Proficiency in Microsoft Windows based Excel, Access and Word.
  • Ability to exercise considerable independent judgment utilizing professional level of knowledge to establish the work priorities within the scope of responsibility.
  • Excellent and demonstrated customer service skills.
  • Case management experience.
  • Ability to work varied schedules which may include weekends/ evenings.
  • Must fulfill all required job qualifications including requirements resulting from current/future modifications to federal/state funding guidelines.
