SleepCharge by Nox Health is seeking compassionate, driven, goal oriented individuals to join our team of telehealth Patient Care Managers. As a Patient Care Manager you will telephonically support patients through their entire SleepCharge treatment plan by providing Positive Airway Pressure (PAP) therapy instructions, support for mask fit and pressure comfort, as well as therapy adherence support and monitoring of medical data. We recommend behavioral solutions to our patients in order to improve their health and reduce their risks of developing serious medical conditions associated with untreated sleep disorders.

The ideal candidate will be flexible and possess the ability to make sound judgments. Applicants must be available for shift assignment Monday through Friday from 2:00 p.m. to 10:00 p.m. EST. Must be comfortable working flexible shifts, especially during training.

Principle Responsibilities:

  • Second shift - 2pm - 10pm Eastern time.
  • Assess and counsel patients who experience technical, medical and/or psychological barriers that impact their treatment usage
  • Provide ongoing support and monitoring to program patients
  • Periodically provide supplies to patients
  • Provide liaison services between patients and medical professionals to ensure timely and appropriate resolution of medical issues
  • Actively engage in team troubleshooting of issues that affect patient care and process changes
  • Perform at least one support function for the benefit of the team
  • Assist in the training of new team members

Education & Experience:

  • Bachelor’s degree or 2 years experience in a healthcare setting or in customer service.
  • Psychology, behavioral science, social work, or other related field preferred
  • Bilingual Preferred (Spanish)

  • Exceptional customer service and interpersonal skills.
  • Ability to prioritize and manage multiple tasks in a high-pressure environment.
  • Possess the ability to work cooperatively as part of a team.
  • Strict attention to detail, organizational skills and follow-through.
  • Critical thinking and problem solving is essential.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only moderate standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Comfortable working with multiple software applications at one time.
  • Ability to learn a proprietary technology platform for medical charting and tasking.
  • Google Drive applications experience helpful.

Pay: $18.00 - $20.00 per hour
