THE POSITION:

Position Title: Remote Patient Monitoring Care Coordinator

Location: Plymouth, MI

Reports to: VP of Patient Experience

FLSA Status: Salary

Date Prepared: July 26, 2022

THE POSITION:

This role will handle in person program inquiries, enrollment, and onboarding of patients to the Medesto Health Platform. Once patients have enrolled, this position will be responsible for:

  • Onboarding education for monitoring device and platform
  • Building relationships with patients for engagement, support and communication
  • Remotely monitoring enrolled patients through daily review of electronic patient data (adherence, physiological, response to therapy, patient messages, etc.)
  • Providing education and other tools to support improved patient outcomes
  • Virtual monthly patient check-ins and reviews to ensure goal achievement and continuity of care
  • Communication between provider and other members of the care team, as needed

  • A solid base understanding of onboarding successes and patient communication
  • Capability to collaborate with technical product experts to resolve patient concerns on a timely basis
  • Ability to work well under pressure in a fast-paced work environment and under tight deadlines

RESPONSIBILITIES:

  • Handle inbound and outbound calls for patients enrolled in the program, onboard patients, and report patient activities
  • Contact new and recurring patients or caregivers as needed to coordinate care
  • Work with prescribers’ offices to obtain the appropriate documentation such as Order form, prescriber NPI, and diagnosis code(s)
  • Obtain additional information from patients as needed to ensure eligibility for said program which may include but is not limited to, insurance eligibility, geographic location, technical capabilities, etc.
  • Document all communications, details of inquiries and actions taken in patient record.
  • Ensure full patient satisfaction and maintain a friendly, helpful demeanor.

Personal Skills/Attributes/Qualifications:

  • LPN with unrestricted license in MI or Certified Medical Assistant
  • >3+ years of related clinical experience or digital healthcare experience; with emphasis on disease management, patient management, telehealth preferred

Job Type: Full-time

Pay: $45,000.00 - $55,000.00 per year
