Purpose:

The Care Coordinator is responsible for enhancing coordination, removing service barriers, and addressing medical and social needs of children, up to 21 years of age. They are accountable for ensuring the delivery of high quality and comprehensive services in a culturally sensitive and appropriate manner.

Pathways to Success is a State of Illinois initiative to provide comprehensive care coordination that will enhance access to critical behavioral, medical and social services for children with complex behavioral health challenges. The program utilizes the evidenced-informed Care Coordination and Support model. This is a structured approach to care coordination that adheres to required procedures for child and family engagement, individual care planning, identifying and utilizing strengths and natural supports while monitoring progress and fidelity to the required processes. C4 is hiring care coordinators to establish access to high quality home and community-based services for children with significant behavioral health challenges. This System of Care improvement initiative will focus on supporting children's success at home, in school and in their communities. We are hiring on both the Westside and Northside of Chicago, depending on what is best fit for you.

  • Utilize IM+CANS to develop an individualized strengths-based service plan.
  • Complete assessments for assigned clients to determine level of need.
  • Develop care plans to reduce areas of health risk.
  • Assist assigned clients with accessing services to improve health including healthcare, housing, education, nutrition, etc.
  • Assemble a unique Child and Family team for each youth including formal and natural supports involved in the youth and family’s lives.
  • Participate in multi-disciplinary staffings for assigned clients
  • Provide support during episodes of behavioral health crisis.
  • Meet with families and other formal and informal supports in the child's life to coordinate care (many of these meetings will be in-person).
  • Complete documentation of contacts and other information in required electronic systems.
  • Monitors daily alerts for participants entering an emergency room or inpatient hospital processes and other coordinated care interfaces, following up with clients and facilitating post-discharge appointments.
  • Provide navigation and warm linkage to resources that address health, housing, vocations, mental health, substance misuse, food insecurity, education deficits, disability needs and other resources
  • Initiate, cultivate and maintain professional relationships with human services and government agencies, health service vendors, and public and private groups to enhance service delivery and C4s presence in the community
  • Frequently make contact to assess a family's progress in meeting goals identified on the care plan

Minimum Qualifications:

  • Must meet Illinois requirements of a Mental Health Professional which can be met with a Bachelor's Degree in an eligible human service degree field.
  • Familiar with and knowledgeable of the communities served (either North or West Chicago)
  • Personal vehicle, valid drivers license, insurable driving record and vehicle insurance
  • Experience working in healthcare or social service agencies preferred

Job Type: Full-time

Pay: From $45,000.00 per year
