*Job Summary*
Under the direction of the Director, Ambulance Services, the Mobile Healthcare-Paramedic is responsible for the care and management of patients seen under the Mobile Integrated Healthcare program and will be responsible for assessment, interaction, and treatment of those patients, including facilitating continuity of care by interacting with their physician and healthcare facilities. Provides primary care and/or advanced life support, including medical evaluation, treatment and stabilization of the critically ill and injured with the goal of reducing morbidity. Supports existing health services; provides integrated health services in partnership with other health professionals; extends access to health services delivery in underserved and general populations, including primary care, public health, disease management, prevention and wellness, mental health, and dental health; and performs other duties as required. The Mobile Healthcare-Paramedic works cooperatively within the department, with physicians and referring providers to create a system of quality healthcare. Functions alone and as a member of a healthcare team providing assessment and care to patients with chronic medical and behavioral problems. Identifies patient’s needs and assists in connecting patients with available community and medical resources. This role will also be responsible for participating in care and treatment modalities as directed by the Department Director and Medical Director(s) including additional operations support functions as required. Work is performed under limited supervision and requires high self-motivation, work ethic, and attention to detail.
Key Responsibilities

  • Performs home visits (scheduled, unscheduled, urgent), assessments and treatments according to MIH protocols and procedures. Responds to patient’s needs, provides assessment, treatments and interface with clinicians in the patient’s home.
  • Must be able to function in a non-traditional setting and provide non-emergency care with a long-term focus.
  • Must be able to function in a traditional setting as a field paramedic providing ALS care activities.
  • Works in a collaborative and team-oriented manner and is able to effectively communicate and work effectively and efficiently with others. Follows operational flow and procedures efficiently and effectively to maximize utilization.
  • Meets established performance improvement clinical benchmarks.
  • Must assist in the development of processes and education materials pertaining to Mobile Integrated Healthcare.
  • Functions as a facilitator and educator for clinical programs including continuing education for field crews and new hires.
  • Drive performance management; assures delivery of high-quality patient care and customer service;
  • Utilize and maintains professional communication skills with all employees, and customers, including patients, facilities and other service agencies;
  • Attends and participates in clinical, operational and strategic planning activities as required;
  • Maintains and keeps all records as required;
  • Prepares written reports as required;

Supervisory/Managerial Responsibilities (include number and type of FTEs)

  • Maintains effective scene management/leadership; follows ICS protocols

Education/Degree
Required

  • High School Graduate or GED equivalent.

Preferred

  • Associates degree in emergency medical services, healthcare or business administration, or other related degree, equivalent experience.
  • EMT I/C or equivalent instructor experience.
  • Instructor certification in ACLS, PALS or related pediatric course, AMLS, CPR
  • Previous educator experience

Experience
Required

  • Minimum of 3 years’ experience as a field paramedic.

Required

  • Experience as a Mobile Healthcare Paramedic preferred
  • FCCS or equivalent experience preferred.
  • IFT or Critical Care experience desired
  • Work independently, be self-directed and contribute as a member of a team

Licensure/Certifications
Required

  • All mobile healthcare provides must successfully complete didactic and clinical education as established by the Program Director.
  • Current MA EMT-P certification.
  • Current Healthcare Provider CPR certification
  • Current Advanced Cardiac Life Support (ACLS) certification
  • Valid driver’s license with no driving violations.
  • Driving record driving record in compliance with department policy.

Preferred

  • PALS, AMLS, PEPP, Community Paramedic certification.

Skills:

  • Ability to think clearly and make quick decisions in an emergency; sound decision making
  • Ability to manage sensitive information in a discreet and professional manner.
  • Ability to perform multiple tasks at the same time;
