An Orthpaedic Care Navigator is needed to manage the patient through pre-operative assessment, education, post-operative discharge planning and services, and communicate with facilities and post-op providers, including skilled nursing and inpatient rehabilitation, to ensure compliance with the procedure protocols developed by FOIs clinical team. This position works remotely from home, but will spend about 50% of their time traveling to various facilities in the surrounding counties of Tampa Bay. Must be able to work on weekends based on call schedule.

Position Description: The Orthopaedic Care Navigator will work collaboratively as part of a multi-disciplinary team to provide patient and family education, communication with health care providers and coordination and facilitation of services within an episode of care.

Key Responsibilities:

  • Responsible for managing a group of patient population throughout a ninety-day episode of care
  • Gather clinical information from the EMR and telephonic health assessments to determine risk stratification
  • Document clinical assessments and care plans
  • Educate the patient and caregiver of the process and expectations of the health care delivery system throughout the course of care
  • Address patient conditions to optimize patients surgery and disposition
  • Collaborate with multidisciplinary team members in the acute, post-acute and outpatient settings to identify patient needs such as home health care, DME, physical therapy and community services
  • Follow up on patients at pre-determined intervals and as needed, based on patient status, telephonic and on-site
  • Complete and submit all required documentation within established guidelines
  • Evaluate plan of care and revise on ongoing basis as determined by patient's health status and communicate changes to all other disciplines involved in the patient's care.
  • Identify gaps or barriers in treatment plans to ensure that discharged patients receive appropriate resources and services
  • Remain flexible and responsive when changes occur in patient activity and workload
  • Work in various systems to enter data, obtain information and research data
  • Adhere to the scope of practice for medical license, if applicable, per state regulatory guidelines
  • Maintain confidentiality of all patient information per HIPAA and Company guidelines
  • Able to travel to local hospitals, Skilled Nursing Facilities and clinics
  • Attend meetings, some after business hours
  • Assure compliance with all company plans, policies and procedures set forth by the Florida Orthopaedic Institute
  • All other duties as assigned.

  • Bachelors degree; 4-year degree in related field preferred
  • Medical Assistant or Nursing certification required
  • Minimum of 2-3 years relevant healthcare or clinical care coordination experience preferred
  • Knowledge of human anatomy and medical terminology; Orthopaedic background is a plus
