  • Current Healthcare Provider BLS Certification
  • Current Healthcare Provider ACLS Certification (must be obtained within six months of employment)

ESSENTIAL FUNCTIONS

1.Assesses and diagnoses patient and family needs in order to provide quality care to assigned patients.

a.Performs admission assessment within eight hours of admission or in accordance with specific unit standards.

b.Identifies and documents nursing diagnosis on patient’s plan of care within eight hours of admission.

c.Identifies and documents patient/family/significant other teaching needs upon admission.

d.Identifies discharge planning needs of patient/family/significant other of admission.

e.Verification of patient identity.

f.Verification of the operative procedure, operative site, surgeon, and consents.

g.Assesses psychological status, including level of understanding, perception of surgery, and anxiety level.

h.Verification of patient’s allergies, NPO status and whether or not patient has voided.

i.Checks the current history and physical, laboratory results, the execution of perioperative orders, and results of ordered diagnostic tests.

2.Develops, discusses, and communicates a realistic problem list (plan of care) for each patient in collaboration with each patient/family/significant other to address all identified needs.

1. Plan of care will include nursing diagnosis statement for each identified problem.
2. Develops patient/family/significant other teaching and discharge plan as per unit standard.
3. Follows procedures for surgical interventions along with preferences of surgeon.
4. Assembles necessary instruments, supplies, and equipment prior to start of case.
5. Checks for availability and proper condition of necessary instruments, supplies, and equipment.

3.Demonstrates the skills and judgment necessary to implement medical plan of care, nursing interventions and procedures as necessary for the care of the patient.

1. Demonstrates knowledge of hospital policies and procedures for administering, transcribing, and recording medications.
2. Demonstrates knowledge of commonly used meds: action, dose, and side effects.
3. Observes five R's for administering meds: right patient, right drug, right dose, right method, and right time.
4. Demonstrates proper procedures in all methods of medication administration.
5. Assesses and documents IV site q4oor per unit standard.
6. Identifies priorities and executes nursing care accordingly.
7. Provides emotional support and comfort measures.
8. Gives information or explanation to patients and their families.
9. Applies and monitors principles of aseptic technique throughout the intraoperative period.
10. Acts as a scrub/circulating nurse during the intraoperative phase.
11. Ensures safe transfer/transport of patient to and from operating room.
12. Performs sponge, needle, and instrument counts according to policy and procedure.
13. Ensures safety of patients during the use of electrical equipment.
14. Preps the surgical site using aseptic technique.
15. Correctly collects and accurately labels specimens.
16. Confines and contains sources of contaminations.
17. Revises physician's preference cards and communicates changes to appropriate personnel.

4.Evaluates the identified problems, care provided, and patient’s responses are reassessed in order to meet patient and family needs to ensure overall quality of care delivered.

a.Reassessment of plan of care is done every 24 hours or per unit standard.

b.Assesses the patient/significant other’s response to teaching plan in order to evaluate the effectiveness of the plan.

c.Based on reassessment, plan of care will remain the same or be modified to meet the patient/significant other’s identified needs.

d.Documents outcomes of patient care.

5.Maintains up to date and accurate documentation of nursing care provided to ensure the integration of information for use by the health care team, to ensure quality care.

a.Nursing assessment is complete and accurate within eight hours of admission or per units standard.

b.Nursing documentation is reflected in the integrated progress notes with an entry made at least every 24 hours per standard and when reassessment of the patient requires.

c.Nursing rounds reports reflect current plan for the patient and shift updates.

d.Discharge plan accurately reflects patient and/or significant other’s ability to manage care after discharge.Referrals are made based on identified needs.

e.Demonstrates proper documentation of narcotics.

f.Accurately and promptly documents unusual occurrences noted during the perioperative phase.

g.All areas in the Intraop documentation record are complete.

6.Maintains current knowledge/certification/licensure. Pursues professional growth and development.

a.Practices within legal boundaries of nurse practice act and according to hospital policy and procedures.

b.Attends continuing education programs in order to maintain licensure. Obtains at least five contact hours per year in your area of practice.

c.Attends hospital sponsored training programs as required by department.

d.Maintains CPR certification.

7.Functions as a role model for current and new staff.

a.Accepts assignment as preceptor for new staff.

b.Facilitates problem resolution among peers as observed by nurse manager and peers.

c.Demonstrates a constructive approach during all interactions with staff and nurse manager toward
the organization.

8.Performs all responsibilities/duties required by the perioperative unit as defined in the scope of service, to ensure that the unique nature of the client is addressed.

9.Attends all required safety training programs and can describe his/her responsibilities related to general safety, department/service safety, and specific job related hazards.

a.All required safety education programs have been attended.

b.Provides complete and accurate responses to safety questions.

c.Operates assigned equipment and performs all procedures in a safe manner as instructed.

d.Maintains work area and equipment in condition required by department standards.

e.Demonstrates proper body mechanics in all functions.

10.Follows the hospital exposure control plans/bloodborne and airborne pathogens.

a.Demonstrates knowledge of techniques, procedures, and correct use of protective barrier equipment.
