On-Call Assessment Coordinator Details:

• PRN availability
  • Schedule is flexible
  • Shifts are on an on basis call 6-12 hours per shift

• Complete remote Clinical Assessments of patients needed to be seen in our partner hospitals
• Facilitate admission or safety planning of patients as needed
• Work from home option
• Review patient referral clinical collateral information from area hospitals for possible admission to PCBH services
  

• During the on call hours you will be paid $2 an hour
• Assesments are $150 plus your mileage

Job Requirements:

• REQUIRED live wihtin an hour of Central Valley Medical Center
• The Admissions Coordinator must have an RN Degree or a Master's in Social Work, LMFT, CMHC
• Customer service oriented
• Excellent communication skills, both written and oral
• Thoroughness and attention to detail
• Comfortable using a computer
• Adaptable and willing to jump in and help as needed
• Able to work productively when the pace is fast or when it slows down
• Spanish helpful, but not required
