MyCare Medical is a Primary Care Group that has been serving several communities across the state of Florida for the last 40 years. We specialize in caring for older adults and keeping them healthy and transitioning them to healthy active seniors. We currently serve over 45,000 Medicare Advantage patients across Florida and Texas in 47 locations and growing.

We recently designed and launched our High-Intensity Program, a program for our high-risk patients who need something extra. Our goal with this program is to reduce hospital admissions, improve the quality of life for our patients and provide seamless care transition. The High-Intensity Program is geared towards our patients with 2 or more hospital admissions or urgent care visits within 12 months.

We are actively seeking a seasoned, autonomous Nurse Practitioner to provide in-person home visits to residents of Pinellas as part of our High-Intensity Program.

ESSENTIAL RESPONSIBILITIES

  • This role will report to our High-Intensity Program Medical Director, seeing our patients in their homes. Our expectations that the person in this role would see approximately 5-7 pts per day.
  • Perform home visits assessments and procedures utilizing your extensive nursing experience and critical judgment skills.
  • Participate in ongoing interdisciplinary assessment of patients.
  • Participate in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s)
  • Counsel and educate the patient/client and family in meeting healthcare related needs.
  • You will be supported by our Medical Director, Social Workers and Care Managers as a core care member on the High-Risk Program.
  • Complete documentation and billing in eClinical Works in accordance with Medicare and practice standards.
  • Performs duties as assigned.

EDUCATION & QUALIFICATIONS

  • Certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC)
  • Currently licensed as an Acute, Adult/Gerontological or Family Nurse Practitioner in the state of practice or the ability to obtain a license prior to employment
  • Holds or is eligible for DEA licensure and CDS licensure (if required in the state of practice)
  • Up-to-date CPR Certification and Basic Cardiac Life Support certification.

· Strong organizational and prioritization skills.

· Ability to use Electronic Medical Record (EMR) system – specifically ECW

· Knowledge of Medical Advantage, HEIDS, MRA and ICD-10 Coding

· Comfortable managing a primarily Medicare-aged patient population

· Demonstrated ability to problem-solve complex and multifaceted situations.

MyCare Medical is seeking an APRN, someone who wants to practice medicine with a focus on patient care. We want our clinicians to take the time needed to truly address the patient’s needs.

ESSENTIAL RESPONSIBILITIES

  • Demonstrates both autonomy and proficiency in performance of comprehensive health assessments.
  • Collaborates with member’s primary care physician to create a care plan focused on prevention, early detection and timely intervention.
  • Observes the scope of practice in the state in which he or she is licensed and working in.
  • Communicates with physicians and family members during transitions of care.
  • Effectively communicates with all members of the care team and facilitates a medically appropriate and effective plan of care to avoid unnecessary duplication of testing, and fragmentation of care.
  • Performs duties as assigned.
