Stratford VNA is looking for an RN to join our intake team and become part of a nationally recognized Home Care Elite agency. Stratford VNA has been providing skilled in-home health care services in Fairfield and New Haven counties since 1919 as an independent, state licensed home care agency, and has been recognized as a Hearst Media Top Workplace by our employees the last four years running. Intake Department staff are the initial contact with Stratford VNA’s referral sources and are responsible for initiating the intake process, follow-through with the insurance authorization and assuring that patient care services are initiated and all appropriate individuals are informed throughout the process.
Candidates must be:
- Licensed as an RN in the State of Connecticut and have a minimum of two years of nursing experience, home health care experience preferred.
- Possess excellent written and oral communication skills
- Confident and competent with technology and electronic documentation
- Be flexible and cooperative in fulfilling all obligations
- Possess the personality which shows aptness in establishing and maintaining effective working relationships with administrative staff, staff members, representatives of other agencies and others
