COMMUNITY BASED / OUTPATIENT
THERAPIST

REPORTS TO: Director of Behavioral Health Services
CLASSIFICATION: Exempt (1 FTE)
COMPENSATION: $5,200.00 - $7,193.33 per month (DOE)

PROGRAM OVERVIEW

The Behavioral Health Outpatient Program at You Grow Girl! launched in 2014. The program’s team is comprised of therapists, social workers, clinical case managers and student interns who work primarily in partnership with schools throughout King, Pierce, Snohomish and Thurston Counties.
Team members also see individuals and families either at their homes, in the community, or our offices in the Southeast Seattle and Kent neighborhoods. As a state licensed behavioral health entity, the program is authorized to provide individual, family and group outpatient treatment as well as case management. The program serves predominantly female-identifying youth, between 11-24 years old, who are in need of support services to live successfully in the community, and their families.

Most of the individuals served are low-income and their services are paid for by Medicaid. You Grow
Girl! does not currently bill private insurance companies but offer sliding scale fees.

Within this program, You Grow Girl! launched Wraparound with Intensive Services (WISe) services in 2019. WISe is a team-based planning process for children and youth, between 5-20 years old, with complex or high-risk behavioral health needs and their families. This service is designed to provide
whole youth, whole family’ support to build resiliency so that the youth can function more effectively and safely in the home with their family, school, and community settings communities.

▪ A team of individuals who are relevant to the well-being of a youth (i.e., family members, service providers, school staff, community members, and natural supports) will be developed;

▪ This team will collaboratively develop and implement an individualized plan of care, monitor the efficacy of the plan, and work towards success over time.

The position will serve participants who enroll or are eligible for WISe services. The Therapist will work collaboratively within an internal WISe care team comprised of a youth peer support specialist, caregiver peer support specialist, and team coordinator, to coordinate interventions to model sustainable collaboration when working with the youth, their family and the external WISe team.

POSITION SUMMARY

Under the supervision of the Director of Behavioral Health Services, the Therapist will be responsible for providing consistent community-based individual, family, and group counseling as well as integrated case management to participants, in person and using approved telehealth methods (i.e.,
Zoom). The Therapist will use strong assessment and therapeutic skills to develop, organize, and facilitate therapy sessions. In collaboration with the participant, the Therapist will be responsible for the development and maintenance of an individualized treatment and/or family care plan(s).

The Therapist will provide clinical guidance and recommendations to other internal and external members of the participant’s identified care team and coordinate treatment interventions with other providers, collaterals and natural supports as needed. The Therapist is responsible for modeling and promoting collaboration among participants, family, natural supports, team members and the community at large to support the youth and family’s treatment goals throughout the process using evidenced-based interventions, psychoeducation and case management.

ESSENTIAL RESPONSIBILITIES

▪ Provide a minimum of 3 hours of direct service activities to every participant each month. This includes face-to-face (in person and/or telehealth) individual, family or group sessions and participation in wraparound team meetings with/for participants hosted by You Grow Girl! and/or external providers (e.g., school, legal, medical, housing, etc.).

▪ Provide a minimum of 3 collateral contacts through in person, telephone and/or telehealth activities with third-party providers (non-relative) to every participant each month.

▪ Establish therapeutic relationships, assess complex problems, select evidence-based interventions and treatment modalities, and inspire participants to function effectively.

▪ Conduct comprehensive assessments, every 6-months or as needed, to determine the appropriate level of care, stage of service delivery, and referrals needed to ensure all life domains are addressed in the individualized treatment and/or family care plan(s).

▪ Coordinate access to the recommended medical treatment services, vocational, employment, housing, and other targeted services. Responsible for information, referral, and timely follow- up to ensure successful linkage.

▪ Coordinates, conducts or participates in assessments; provides crisis intervention & management assistance services; delegates recovery plan tasks to appropriate internal or external service providers; and monitors overall service delivery.

▪ Facilitate or co-facilitate at least one (1) psychoeducation, skill-building or therapeutic group, workshop or training each quarter.

▪ Act as a liaison and advocate on behalf of participants and their families by providing efficient and quality case management and care coordination.

▪ Ensure 85% accuracies and timely submission of all participant-related administrative tasks and documentation, including progress notes, 6-month assessments, treatment/care plan(s), and crisis plans and other reporting paperwork by specified due dates to meet contractual requirements of our stakeholders (i.e., HCA, ProviderOne, MCOs, etc.).

▪ Participate in on call crisis after-hour phone rotation between the hours of 12:00am to 6:00am

PST; two (2) days a month.

▪ Maintain a caseload of no less than 12; no more than 19 participants.

▪ Participate in required supervisions, meetings, consultations, administrative in-service days, and professional development trainings on time and for the full duration.

SKILLS & EXPERIENCE

▪ Ability to maintain a healthy ‘ work-life-school-self’ balance through the development of and consistent self-care practice, which is to include asserting therapeutic boundaries with participants and their collaterals and engaging in therapy or self-healing/resetting activities.

▪ Ability to learn and to utilize telehealth platforms, such as FaceTime and Zoom.

▪ Skills, experience, and ability to provide ethical, effective, and culturally responsive services to diverse populations, specifically black and brown female-identifying youth and their families.

▪ Understanding and awareness of issues commonly affecting female-identifying youth.

▪ Experience and ability to use evidence-based interventions and treatment modalities.

▪ Maintain confidentiality in accordance with 42 CFT Part 2, HIPAA, and internal policies.

▪ Ability to keep and maintain up to date files with appropriate and thoughtful documentation.

▪ Ability to meet explicit deadlines and paperwork requirements.

▪ Ability to function individually and as an active member of a multidisciplinary team.

▪ Ability to assist with mental health crisis situations during scheduled working hours.

▪ Ability to learn and to utilize electronic data systems and case record documentation.

▪ Ability to learn and to utilize Microsoft Office Suites, specifically Outlook desktop version.

▪ Ability to maintain professional behaviors and appropriate boundaries with participants.

▪ Ability to complete tasks as outlined in this job description or assigned within alignment of scope of practice by administrators by specified due dates.

▪ Ability to learn and to comply with all written internal policies and procedures in relation to your responsibilities as a healthcare professional and employee.

LICENSES & OTHER REQUIREMENTS

▪ At minimum a bachelor’s degree in social work, counseling, psychology, sociology, or related field with extensive clinical work experience. BSW or master’s degree strongly preferred.

▪ At least three years of experience working with systems-involved children, youth or adults.
Clinical internships and volunteer experience will be considered.

▪ Current Washington State Department of Health professional credentials, as an Agency

Affiliated Counselor at minimum.

▪ Must be able to perform duties independently and collaboratively, ethically, effectively, and efficiently both in person and using telehealth methods.

▪ Must possess a valid Washington State Driver's License. If using own vehicle for work purposes must provide and maintain proof of insurance. Willingness to engage in frequent travel throughout service area is required for this position. Position is not fully remote.

▪ Obtain Child and Adolescent Needs and Strengths (CANS) certification within 30 days of hire.

▪ Ability to pass a local and national Criminal Background Check.
