· Complete service delivery so that client needs, program goals and contract objectives are effectively met.

· Contribute to agencies core values by providing quality results.

· Coordinate with community agencies and attend closure or conference staffing.

· Complete all documentation and reports in a professional and timely manner

· Maintain and generate necessary reports in a professional and timely manner

· Provide Targeted Case Management services including group counseling, individual counseling and social skills training and documentation.

· Participate in treatment team meetings

· Maintain appropriate documentation of all Targeted Case Management services.

· Complete work assignments so that service delivery is performed according to agencies mission, policies and procedure and core values.

· Participates in case consultations and in-service training as appropriate.

· Identify and assess client and family needs and evaluate, coordinate and oversee necessary service and/or treatment is provided; provide in-home services to clients, complete required assessments; assist and counsel individuals and families helping to articulate goals and providing needed information.

· Service Location will temporarily be performed in the office, through Tele-health or in the community instead of client’s homes due to Covid-19 virus.
