PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Metro LA Outreach team as the Mental Health Specialist at the Brookfield location.

ABOUT OUR TEAM
PATH’s Street Outreach programs utilize our street outreach integrated approach to engage with individuals who are currently experiencing homelessness and living on the streets. The goal of these programs is to quickly assess and link individuals with the supportive services and resources necessary to improve their well-being and quality of life, transition off of the streets and obtain and sustain housing long-term.

WHAT IS A MENTAL HEALTH SPECIALIST
As part of the Metro LA Outreach team, the Mental Health Specialist work alongside a Multidisciplinary Street Based Engagement team comprised of case managers, housing locators, and community providers to provide field-based, direct client services.

Position Responsibilities:

• Conducting daily street outreach to engage and build rapport with clients experiencing homelessness
• Conducting and advising on crisis and mental health assessments and interventions in the field
• Coordinating with community mental health and substance use treatment providers to link clients to urgent and long-term services
• Liaising with law enforcement and local hospitals around involuntary hospitalizations and safety issues
• Providing transportation and accompanying clients and their children when necessary to access to medical care, obtain shelter, and attend appointments to provide support, advocacy, and assistance
• Responding to requests for outreach services
• Providing street case management services designed to connect clients to long-term supportive services including:Assisting with documentation gathering, such as obtaining identification documents and documentation necessary to obtain housing

Employment assistance
Housing referrals, navigation, and application assistance
Assisting with applications for financial and medical benefits
Working with law enforcement and interim service providers to ensure a smooth transition from street living to interim housing for clients

WHAT YOU BRING

• Demonstrate knowledge of and experience with advanced and evidence-based interventions including, but not limited to: Harm Reduction; Motivational Interviewing; Critical Time Intervention; Housing First
• Have experience assisting with SSI and SSDI applications
• Demonstrate the adaptability and flexibility to be excel in a fast-paced and dynamic environment 100% in the field
• Demonstrate strong skills in engagement and rapport building with hard-to-reach clients
• Are comfortable working with and building successful collaborative relationships within the community including with law enforcement, service providers, members of the community, and other stakeholders
• Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
• Work just as well independently as they do on a team

MINIMUM QUALIFICATIONS
To be considered for this position, we require all candidates:

• Master’s degree in behavioral health field that is eligible for licensure through the CA BBS (e.g., MSW, MFT)

MANDATORY REQUIREMENTS
Regardless of qualifications, candidates must:

• Have employment eligibility verification
• Have or be able and willing to obtain CPR/First Aid training
• Provide proof of full COVID-19 vaccination
• Successfully complete the following as a condition of hire:Tuberculosis Test

Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:

• Have reliable transportation
• A valid driver’s license
• Proof of insurance and ownership for personal vehicles used during work duties

COVID-19 VACCINATION
Effective 11/01/2021 PATH will require all employees to be fully vaccinated for COVID-19. All prospective hires will be expected to provide proof of full vaccination as part of the pre-employment credentialing process.

FLEXIBLE WORK
PATH is a California employer. Therefore, all regular and customary work must be performed within the state. Approval for work outside of the office, does not establish approval for work to be conducted outside of California.
