SUMMARY
Provides individual and group counseling to persons receiving medication assistance treatment for opioid and alcohol use disorders. The counselor develops treatment plans with patient, case management, medication monitoring and other interventions when indicated. Completes comprehensive substance use assessments prior to admitting a patient to the program. Leads didactic and process groups. Assess individual and group behavior and provides appropriate behavioral interventions. Implements the intake and treatment plan development processes. Follows hospital wide and department specific policies and procedures in the provision of treatment


ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Provides accurate, objective, non-judgmental observation of patient behavior and documents in patient’s electronic record. Completes in-house assessments promptly. This also includes monitoring patients during group activities. Documentation of admission information, assessment findings patient behavior, and response to treatment must be in clear, concise, objective behavioral terms.
2. Completes Substance Use intake Assessments to determine a patient’s appropriateness for admission to treatment level of care according to ASAM criteria. This includes consulting with Clinical Supervisor before admitting patients with medical problems and before admitting patients referred from medical facilities.
3. Completes pre-admission screenings on patients on medical floors and ED when indicated.
4. Teaches didactic seminars, leads process and discussion groups and implements appropriate individual and group treatment interventions and behavioral consequences.
5. Executes crisis intervention strategies in cases of threatened suicide or violence.
6. Provide orientation to treatment in a manner that encourages continued participation.
7. De-escalates potential conflict while maintaining order in the program. This includes providing problem solving assistance to patient, families, and significant others.
8. Completes outreach activities to help patient remain engaged in treatment, follows up on missed appointments within 24 hours via phone call or home visit and documents results.
8 .Completes treatment plans, continued stay reviews, after care, and discharge plans on a timely basis per policy.
9 .Facilitates appropriate referral consultation based on assessed needs, follow-up of required or delayed testing, care pathway and results of physician intervention.
10. Submits reports to law enforcement, courts, employers, or government entities with required releases in a timely manner.
11. Submits appropriate billing of services provided per organization policy
12. Coordinates discharge planning, including assessment of discharge needs resource availability and communication of patient's needs among team members and family's.
13. Participates in weekly ASAM staffing’s with multi-disciplinary team including Physician, MAT (Medication Assisted Treatment) Counselors, and other Providers when indicated. Communicates utilization issues; discharge planning reviews and variances of care.
14. Demonstrates awareness of regulatory requirements, including but not limited to, SUPR, HFAP, IDPH, and CMS.
15. Treats patients, peers, and other professionals in a respectful manner at all times in any interaction.
16. Actively seeks opportunities to attend job related training activities to enhance professional growth
17. Performs other job related duties as assigned.


JOB REQUIREMENTS:
  • High school diploma or GED equivalent required. Associates or Bachelors preferred.
  • Academic training in chemical dependency preferred, CADC or CSADC required.
  • Two (2) years’ experience in chemical dependency or a combination of chemical dependency and behavioral health preferred.
  • Illinois certification as substance abuse counselor or eligible for such within one year from date of hire. Strong communication skills, utilizes problem-solving process, acts as a team leader, demonstrates good work ethic, demonstrates good nursing assessment skills, maintains confidentiality, demonstrates flexibility and analytical skills.
  • Has working knowledge of resources available in the community for the patient and families.
  • Has clear understanding of ASAM placement level of care criteria.
  • Must demonstrate a working knowledge of Illinois rules 2030, 2060, and 2090 which governs substance abuse treatment in the state of Illinois.
