Position Description:

Responsible for transporting patients to and from scheduled appointments per the transportation schedule and routine maintenance on the vehicle per maintenance schedule. Responsible for running errands for administration. This position requires compliance with CommuniHealth Services written standards, including the Compliance Plan and policies and procedures (including the Code of Conduct).

  • Provides transportation to and from appointments at CommuniHealth clinics, initial referral appointments scheduled by CommuniHealth, and 340B Pharmacies.
  • Obtains supervisor and/or management authorization for all other transports prior to transporting.
  • Obtains transportation usage agreement from patients utilizing CommuniHealth transportation services per Transportation Procedure.
  • Maintains the safety of patient and driver at all times.
  • Ensures all passengers are secured in a seatbelt and/or an age appropriate child safety seat at all times.
  • Assists patients entering and exiting van, when applicable.
  • Maintains transportation usage log per Transportation procedure.
  • Maintains interior and exterior of CommuniHealth transportation vehicles at all times.
  • Maintains fuel level is ½ full at all times.
  • Drives vehicle using standard operating procedures.
  • Facilitates vehicle scheduled and routine maintenance.
  • Responsible for running errands for administration, as needed.
  • Refrains from using CommuniHealth transportation vehicles for personal use unless authorized by management.
  • Bastrop Transportation driver maintains mobile unit mileage log when mobile unit is leased out to other clinics.
  • Marion transportation driver is responsible for delivering mail/supplies from Administration to necessary clinics on Friday of each week.
  • Marion transportation driver is responsible for picking up mail from off-site clinics, secured in a bag to sustain confidentiality, and deliver to CommuniHealth Administrative Services Coordinator, on Friday of each week.
  • Maintains compliance with and knowledge of all CommuniHealth departmental, facility specific and system-wide policy and procedures.
  • Maintains, and is in possession of, a valid Louisiana driver’s license at all times.
  • Maintains a good driving record clear of misdemeanors or felonies.
  • Completes required CHS training per policy, maintains compliance with and knowledge of all CHS departmental, facility specific and system-wide policy and procedure, attends mandatory departmental meetings and/or training, and follows CHS Mission, Vision and Values in delivery of all services.
  • Participates in all appropriate meetings within the organization.
  • Maintains strictest confidentiality and adheres to HIPAA guidelines/regulations as well as compliance with HRSA regulations.
  • This position may require work beyond normal working hours.
  • Performs other duties as may be assigned.

Education:

High school diploma or GED.

Skills/Experience

  • Must be able to read, write and speak the English language in an understandable manner.
  • Current driver’s license with good driving record (no misdemeanors or felony record).
  • Knowledge of vehicle maintenance.
  • Knowledge of traffic laws and regulations.
  • Knowledge of motor vehicle functionality.
  • Skilled in driving light and heavy vehicles.
  • Time management skills.
