Position Responsibilities:

  • Assembles medical records for Eldercare participants.
  • Assembles medical records in a timely manner or between 12-20 minutes per chart.
  • Answers telephone calls by third ring with appropriate department greeting, addressing caller in professional manner and operations excellence standards; records accurate information.
  • Assures that every record contains a demographic face sheet, by printing and inserting face sheets when necessary.
  • Verifies that participant's name and medical record number is correct on all documents, including folders, reports and bar code labels.
  • Uploads documents into chart using correct order, naming, date and reviewer. Accuracy >/= 95%
  • Discards copies when appropriate in shred it bins to promote confidentiality of participant records.
  • Consistently demonstrates the ability to recognize, establish and deal with priorities in a timely manner without sacrificing quality of work.
  • Manages time efficiently to insure that all work is completed in a timely manner; utilizes any extra time to assist with other department duties as needed, such as preparing enrollment folders, stocking participant educational material, etc.
  • Communicates procedural or problem areas to manager to ensure continuity of department workflow.
  • Signs medical records in/out accurately utilizing chart tracking system. Accuracy Rate = 100%
  • Demonstrates ability to locate difficult to find medical record items in charts.
  • Follow Occupational Health guidelines to ensure proper registration and prepping of clients charts.
  • All documents received via fax, mail or hand delivered are uploaded into EMR on the day received.
  • Provides and explains all registration document (Enrollment Agreement, HIPAA Information, participant Rights Information). Obtains all appropriate required signatures as a back up to the Enrollment Team.
  • Audits and closes the EMR for expired or disenrolled participants.
  • Accurately enters DME, medical supply items into EMR.
  • Responds to internal and external requests for medical records and documents each request per Eldercare policy.
  • Tracks requests for medical records and follow up per Eldercare policy.
  • Prepares and sends medical records for external specialists appointments, ER and hospital stays.
  • Attends IDT meetings and follows up on all requests for medical records.
  • Identifies and tracks all outside appointments and requests documentation for the encounter. This incldues labs, imaging, hospital stays and physician encounters.
  • Participate in and support Quality Improvement initiatives
  • Know and adhere to philosophy and goals of Eldercare

Qualifications:

  • Registered Health Information Technician (RHIT) Preferred
  • High School Graduate or Equivalent
  • One year of medical records experience preferred. Knowledge of electronic medical record management (EMR) systems preferred. One year of experience working with a frail or elderly population.
