Under the general direction of the Administrator, the Clinical Nurse Manager - SNU provides clinical and quality resident care to ensure advancement of clinical practice and resident care. Develops, implements, and manages ongoing quality measurements, action, planning and evaluation methods related to productivity, culture of safety, colleague engagement, resident satisfaction, physician satisfaction and clinical outcome targets. Responsible for oversight of resident assessment utilizing the RAI and IDT process. Is also responsible for oversight of the admission, quarterly and annual assessments, significant change PPS assessments, reassessments, significant corrections and return stay/re-admission assessments. Direct reports for this position are the RN’s, CMA’s, CNAs, LPN's and MDS position. This position assists in overall functioning of the unit, especially regarding education and regulatory aspects. What you will do:

  • Demonstrates the knowledge and skills necessary to provide service/care appropriate to the age of the patients served, in accordance with MercyOne standards.
  • Demonstrates the knowledge with the MDS/CAA/Care planning process based on RAI manual.
  • Demonstrates knowledge and skills to extract data from resident's active medical record for accurate coding of MDS and provides oversight to the MDS position.
  • Reviews ADL charting by C.N.A.s and Nursing, provides education on correct coding of ADLs based on RAI manual. Recommends and works with the Lead CNAs with development of education for CNAs ADL charting.
  • Reviews MDS/CAA/Care planning; charting and assists in updating care plans.
  • Provides nursing education with accurate charting. Acts as a mentor and serves as clinical resource person to colleagues.
  • Oversees the coordination, development and completion of the residents' (MDS) in accordance with all Federal and State guidelines.
  • Oversees timeliness and accuracy of MDS submissions; monitors timely completion of assessments by other departments involved in this process.
  • Demonstrates proficiency in MDS software; trouble shoots clinical software issues;
  • Works closely with MDS staff and therapy to ensure maximum PDPM; Observes Rehab assistants performing ADL's.
  • Educates potential customers about skilled care services, expectation and routines of the unit.
  • Assists Administrator with reviewing and revising policies, procedure and standards.
  • Works with MDS staff and Social Services to assure an opportunity for family/responsible person(s) and resident to participate in the care planning process;
  • Provides competent resident care to the above-indicated resident populations by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatments.
  • Takes an active role in enhancing ability to carry out job functions through personal and professional/job-related growth and development, and participates in MercyOne Siouxland Medical Center and/or departmental educational programs;
  • Maintains a working knowledge of applicable Federal, State, and local laws and regulations, MercyOne and Trinity Health's Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior;
  • Must have or be willing to obtain CMS Infection Preventionist designation to oversee the IC and ASP policies in coordination with the Hospital IC program
  • Will be responsible for the management of Nursing, CNA and MDS colleagues; duties include all responsibilities to assist in proper unit staffing.

What we are looking for:

  • B.S.N. degree required.
  • Must have an active Iowa RN license in good standing;
  • Must be knowledgeable with regards to laws, regulations and guidelines that pertain to long term care.
  • Must have excellent organizational abilities.
  • Must have excellent assessment skills.
  • Basic Life Support Certification is required.
  • Excellent interpersonal/listening skills to interact effectively with individuals of all backgrounds, experience and educational levels.
  • Excellent written/oral communication skills. Demonstrates ability to communicate effectively with staff, physicians, and other allied health professionals.
  • Individual should be dynamic, creative and intelligent.
  • Successful individual should be a mature executive with a professional appearance/demeanor, excellent character, integrity and commitment.
  • Must be able to function effectively under stress.
  • May be requested to travel within the Siouxland region. When assigned to travel, a valid driver’s license and proof of insurance will be submitted to manager prior to travel.
