Job Summary:

We are currently seeking exceptional Licensed Practical Nurses (LPNs) for our Community in Schererville. This is a senior living community with 85 Independent Living, 38 Assisted Living and 57 Memory Care apartments. We embrace a culture of hospitality in providing excellent service, care and support for our residents. Our residents are the reason we do what we do every day!

We are looking for experienced LPNs that will be responsible for providing nursing care according to physician orders, resident assessments and care plans, recognized nursing practice, established standards and administrative policies.

Must be able to work a flexible schedule including every other weekend and some holidays. Previous experience in geriatrics, Assisted Living and Memory Care preferred.

Essential Job Functions:

• Provide direct nursing care to assigned residents and provide daily supervision to staff.
• Assist residents with Activities of Daily Living, family/social activities and actively promote an independent life-style for residents.
• Assess and monitor complex resident health status and determine the appropriate nursing intervention. Develop and monitor the ongoing plan of care for residents and initiate corrective action as necessary.
• Ensure preventative, therapeutic and rehabilitative services to residents, and assist in the coordination of care and services provided by other departments.
• Accurately administer prescribed medications, supervise the continuity of nursing care, respond to changes in medical condition, medications and other circumstances requiring special care needs; and perform first aid as required.
• Assure that all resident records are accurate, complete all forms and documentation as required. Also assure community policies are being followed, that nursing care is practiced in full compliance with state regulations, our facility standards and resident rights.

Requirements:

1) One year LPN experience -- in a skilled nursing, Assisted Living or Memory Care setting preferred

2) Current Indiana Nursing License

3) CPR Certification

4) A strong desire to work with the elderly and provide outstanding care, service and support.
