Job Summary

The LPN is responsible for rendering nursing care in assigned area and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing. Supervises staff as assigned.

Position Responsibilities/Standards

  • Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned resident as evidenced by documentation and observation of positive resident care outcomes.
  • Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards.
  • Assist in data collection on the Admissions Data Record.
  • Contribute to the development of the resident’s plan of care based on the identified resident needs.
  • Assist in the evaluation of the effectiveness of the nursing care administered relative to the resident’s identified needs.
  • Document pertinent observation and intervention on appropriate record.
  • Perform selected tasks related to resident care and the resident’s environment in a safe manner.
  • Administer medication accurately, observing resident response, as evidenced by documentation in the medical record and lack of negative outcomes. Perform limited I.V. Therapy procedures on those residents receiving peripherally inserted Intravenous fluids, as delegated and observed by the RN.
  • Perform limited I.V. Therapy procedures upon proof of successful completion of Tennessee Board of Nursing approved Intravenous Therapy course, as observed by Department Director and RN.
  • Maintain and coordinate communication among residents, staff, and physician as evidenced within medical record, resident, and health care team feedback.
  • Assist in assuring that effective communication exists with resident/family as reflected in the plan of care.
  • Give punctual change of shift report consisting of pertinent and relevant information for assigned residents.
  • Effectively communicate residents changing needs and plan of care among staff members.
  • Report resident needs/problems and observations regarding condition to the Registered Nurse and physician.
  • Reinforce resident/family information regarding plan of care and discharge plans.
  • Work collaboratively with other members of the health care team in coordination of the resident’s care as evidenced by other department feedback.
  • Provide resident/family education as delegated and observed by the RN.
  • Demonstrate flexibility to perform duties wherever volume deems it necessary with the Organization.

Personal Development

  • Attend required annual in-service programs.
  • Participate in continuing education activities and obtain required contact hours.

Required:

Currently licensed LPN in this State. Must be actively certified, in good standing, as a LPN in Ohio. (If certified in another state, must be seeking reciprocity with Ohio.)

Certified in Basic Life Support.

Preferred:

Minimum of 1 year nursing experience.

Previous experience in long-term care setting

Previous charge and/or supervisory experience.
