Summary Description of Position: Licensed Practical Nurse position consists of services that help clients gain access to needed medical, social, educational, therapeutic and other services. This includes direct assistance in gaining access to services, coordination of care, oversight of the entire case, and linkage to appropriate services. LPC includes: (1) assessment of the eligible client to determine service needs; (2) development of an individualized integrated care plan; (3) referral and related activities to help the client obtain needed services; (4) monitoring and follow-up; and (5) evaluation.

LPN General Responsibilities:

· Ensure safety of clients and fellow colleagues as a number one priority at all times.

· Protect clients and fellow colleague’s information in regards to confidentiality.

· Maintain a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature.

· Maintain an active caseload providing interventions as needed within area of expertise and limits of credentials; assures procurement of additional services as needed.

· Interview residents, referral resources, and significant others to obtain needed information to ensure accurate treatment planning and discharge planning.

· Conduct random urine drug screens and breathalyzer tests and medication pass assistance

· Provide crisis management for residents; makes linkages for interventions as appropriate.

· Provide clients individual and group Activities of Daily Living (ADL) training.

· Consults and cooperates with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with the focus on attaining treatment goals.

· Conduct client education on addiction, recovery, and/or other areas as needed for provision of client care.

· Maintain detailed and timely reports, records and notes of resident’s progress. Maintains client and program records in accordance with applicable standards and regulations.

· Participate in interdisciplinary treatment planning conferences, resident community meetings, clinical meetings and program meetings.

· Interpret clinical findings and medical recommendations to client, family referral sources and as indicated.

· Maintain contact with all referral sources and participate in referral development activities as needed.

· Attend meetings as appropriate and meet regularly with supervisor to exchange pertinent information and receive supervision.

· Maintain clear and effective notes about the client’s progress and communicate effectively with staff about any needs or concerns regarding the residents.

· Maintain licensure/certification and current clinical/medical competence and uphold professional code of ethics.

· Participate in continuing education activities, remaining knowledgeable in area(s) of expertise.

· Maintain positive and clean work environment for clients and clinical team at all times.

· Cooperate with other employees, including management in a respectful, helpful manner.

· Proactively seek guidance and counsel on how to respond to certain events or circumstances.

· Respond to emergencies in accordance with first aid and CPR training, and good judgment.

· Abstain from alcohol, nicotine, illicit drug, and/or other substance use while on duty, on call or on Employer’s premises.

· Other duties as Employer may assign from time to time.

Care Coordinator Specific Responsibilities:

· History and Physical assessments: Complete during intake process and reviewed by licensed physician who will determine need for medical treatment.

  • Individualized Medical Treatment Plans: Implement per the order(s) of the site physician following initial assessment by medical team.
  • Medication Management as needed
  • Referral to On-Site Services & Community Providers:In support of overall physical well-being during recovery treatment.
  • Progress Assessment of Detoxification: Monitor vital signs, withdrawal symptom management, and physical assessments under direction of site physician during outpatient detoxification and/or changes to medical treatment plan by onsite medical staff.
  • Provide for Continuity of Care
  • Appointment & Transportation Scheduling
  • Individualized Health & Lifestyle Education: Counseling on medication(s), medical diagnosis, chronic care management, lifestyle choices, physical activity, self-care, preventative care access and importance
  • Community Resource & Networking: Support and assist each patient in planning healthy self-care and maintenance of recovery upon discharge
  • Other Supportive Services: In accordance with facility policy and procedure, Care Coordinator will also serve as patient resource for information on and enrollment in on-site and community programs supporting physical activity, personal and professional growth, and volunteerism.

· Access to on-site medical staffing shall be during normal operating hours. Appointments with on-site medical staffing will be made in advance when reasonable. Emergency medical services to be provided by county EMS and per facility policy and protocol.

Certificates, Licenses, Registration:

  • Valid driver's license in accordance with NC DOT.
  • Maintain current American Red Cross CPR/1st Aid certification & Seizure Management
  • Maintain current Least Restrictive Interventions
  • OTHER REQUIRED TRAININGS VIA NCDHHS
  • Medical Licensure

Job Types: Full-time, Part-time, PRN

Pay: $25.00 - $28.00 per hour
