ROLE SUMMARY


The Licensed Practical Nurse (LPN) builds from the resources of the organization & community to provide services to meet the needs of individuals and families within their homes. Under the supervision of an RN, the LPN provides direct nursing care, within the scope of their license, to patients as directed by the physicians and other healthcare providers. Cooperative decision making is expected in the execution of duties.


  • Implement and evaluate patient care under the supervision of an RN. Participates in the development of a patient-specific care plan.
  • Provides direct patient care including administering of medications and treatments as prescribed.
  • Teaches and counsels the patient and family/significant others regarding the nursing care needs according to the plan of care.
  • Regularly reevaluate the patient’s nursing needs, communicating any changes in need or condition to the physician and other necessary clinical staff.

  • Graduate of an accredited school of nursing and licensed in the State of Oregon
  • Experience: Minimum of 1 year experience in a clinical setting, home health/hospice care experience preferred.
  • Licenses/Certificates/Registration: Oregon State Registered Nurse licensure required. American Heart Association BLS required. Current automobile insurance required. Valid Oregon driver’s license and reliable transportation required.
  • Additional requirements: Travel required throughout Partners In Care’s central Oregon area of service coverage including LaPine, Crescent, Christmas Valley, Prineville, Madras, Sisters.
